Projectile vomiting in the morning

The past two mornings my 11 week old has projectile vomited all of her morning feed. She feeds about 2-3 times in the night, I always take care to wind her and keep her upright before putting her down as she has silent reflux, we are on 0.9mls of Ranitidine 3 times a day we've been on this since she was 3 weeks old. It's making little difference. She feeds efficiently and regularly so in no way am I worried she's starving! But obviously it's upsetting to see milk projectile out of her mouth and nose, and sods law it's always immediately after I've changed her and got her ready for the day she vomits over us both! Any ideas what could be causing this and what I can do to help her? HV saw us yesterday as I requested a visit.. she said LO Is just being greedy and not realising she's full until it's too late. I'm worried it'll be a regular thing, her morning and night time feeds are always the longest so it was a lot of milk that came up. She's an ebf baby.. HV also said that her feeding frequency means at the moment I'm just topping her up from her last feed and she doesn't know what it feels like to be really hungry but how can you let your LO cry if they want feeding? She's well stimulated throughout the day, play mat, baby toys, baby groups, and I always let her tell me when she's hungry rather than offering .. any advice would be welcome.

This happened with mine now and again during the first few months. It was really frustrating and sometimes frightening but just stopped over time.

I'm not sure I understand the bit about baby not feeling hunger because you are just topping her up. Was HV suggesting you don't feed her on demand?? That seems like bad advice. BF babies should be fed on demand and especially during the first weeks when you are trying to establish breastfeeding.
