21 week old farting but not pooing

My LG is 21 weeks old, and since being about 12 weeks she has had ongoing tummy troubles. She poos every 2-3 days - and when she does it is explosive and the consistency of milkshake (sorry!) On the days when she doesn't poo, she is very uncomfortable - farting constantly, crunching her legs up and crying lots. She wakes every circa 45 mins through the night, usually in discomfort.
She is breast-fed with a top-up formula bedtime bottle. On taking her to the doctor 4 weeks ago, he thought it might be a cow's milk intolerance so suggested I cut down on my dairy intake and they put her on Nutramigen 1. This made absolutely no difference. I went back to the doctor last week and they have transferred her onto Neocate. They have also told me to eliminate dairy completely, which I have done.
I know that it takes a while for dairy to leave the system, but it has been 10 days with absolutely no dairy at all and I would say her symptoms are as bad as ever (if not slightly worse). She is clearly in a lot of discomfort, and whenever she farts she lets out high-pitched screams.
I have tried warm baths, cycling her legs, massaging her tummy and lots of tummy time but nothing is working. The doctor has said we need to try the Neocate for 4 weeks (together with me being dairy free)…..if no improvement, he will refer her to a paediatrician. My worry is that will be a further few weeks (at least) before we see anyone else, and all this time she is in pain.
I really don't think it is dairy causing the problems, as surely we would have seen just a bit of improvement? Anyone had a similar experience with their little one? Or any other ideas of things I could try to relieve her discomfort? x

Cutting down on dairy wouldn't make any difference, if that was the problem. It would have to be eliminated entirely and I believe it can then take up to six weeks for all dairy to leave both you and DD system.

But, to be honest, not pooing every day is quite normal as is some discomfort while they get used to their digestive system and the frequent night waking could well be the four month sleep regression
