How to stop nappy rash in my 2 y.o.?

2.3 y.o. DS is getting really bad nappy rash - red inflamed skin where it's been in touch with his (cloth) nappy, but not in the very creases of his groin.

He has eczema and generally sensitive skin. He has had allergic dermatitis in the nappy area when we've used various creams (Daktacort, Timodene, Diprobase emollient cream).

The dermatologist suggested stopping washing his bottom with Dermol, which we've done. It improved a patch on his buttocks but the front of the nappy area is getting worse.

I've had to take him out of cloth in the day (Fuzzi Bunz pockets) but his night time nappies are still making his skin inflamed and it's getting worse. We use Little Lamb cotton nappies with a fleece liner, hemp booster and Airflow wrap.

I've not tried strip washing the nappies, but I'd be very surprised if detergent build-up was a problem as I usually wash with soapnuts, which don't leave residue.

I normally do a half to full rinse cycle (20-50 mins) before washing the nappies with soda crystals and soapnuts.

Recently I've tried washing without soda crystals, with bio washing powder (about 1/3 the recommended amount on the box), then doing another half to full wash with plain water afterwards to make sure all the detergent comes out. I can't see any bubbles in the washing machine when I do that. It doesn't seem to have made any difference.

I do a maintenance wash every month or two and I'm pretty sure we don't have problems with detergent / mould in the washing machine.

He gets very wet at night. He bf's before bed and every time he wakes. He drinks plenty in the day.

Any ideas for how I can keep using cloth? I've not yet found a disposable nappy which will last all night, and I hate them anyway. DS uses the potty at home and before bed but isn't ready for full potty training yet, and I reckon he's several years off being dry at night.

When my DD has this problem I realised that it was due to her eating citrus fruit. They made her wee very acidic, which then irritated her skin.
Is there any possibility that something in his diet is the problem rather than the nappy?

Thanks, that's a useful suggestion. He has a nut allergy (which causes eczema flares) and his eczema reacts to things like tomatoes and citrus if it's inflamed.

He's not had citrus lately, but possibly it could be tomato related - or something else. I probably need to keep a food diary really.
