advice needed on going for new job

some of you may recall my (various) dilemmas regarding work in general/staying at home/my work in particular.
Well, my current job is getting less tenable (nothing major, just some mistakes with my salary over xmas, coming on top of the whole company being on 80% salary for 6 months+, plus my losing faith in the board of directors ability to dig the company out of the hole we are in etc etc - actually, sounds quite major now it's written down!)
anyway, in a fit of pique I blasted my cv off to a company yesterday and they've gotten back to me today asking me to detail my mid-term career aspirations and how they fit tthat company, as well as my current salary and expectations and a few other details
so I guess that is good. Problem is I am fairly low in self-esteem/motivation at the mo (after 3 years at current company) and just can't think what to write. I feel like I don't have any career aspirations anymore. Just being paid my salary would be nice.
Can anyone give me a kick up the behind/some sensible advice? what sort of lingo to use etc?
Also, ideally I would like a part time job, that's what I have at mo. Although if the job is right I will consider going f/t and hope to negotiate back to p/t later. However, what to do now - should I mention that (as a career aspiration perhaps ) now? Do I give them my pro-rated salary or full salary?
Oh what to do. Help me please before I go off the boil and end up staying where I am through sheer inertia.....
(this is another SME, I am a bit nervous as experience within SMEs hasn't been great, any tips on warning signs to look out for if I get to interview stage [HA HA] most welcome)

3rdTMT - I have done my email and am about to send it. Your advice was invaluable, especially regarding what to say about F/T vs P/T. I am going to go with your idea of telling them what I do now, but that I'd be ok about moving to F/T for the right role. At least that opens the dialogue at the beginning and I should know early on whether or not they are open to P/T.
