Incident form for bite at nursery

Hi all,
Ds1 recently started nursery and I had a phone call while he was there today saying another child had bit him and that they had applied ice and seemed fine but to keep an eye on it.

I picked him up and he was his happy self but I didn’t receive an incident form. It looked quite a bad bite with bruising in the middle of it so I took him to the gp surgery to see if it needed seeing too. The receptionist said there was no nurses available to look at it but they recommend any bites to be seen at A&E. The pharmacist beside the surgery had a look at it and also recommended it should be looked at by A&E.

I took ds straight there and they looked at it and said he was fine to go home but to keep an eye incase it gets any redder.

I’m just wondering if I should ask for a copy of the incident form tomorrow incase it does need to be seen to again?

You should definitely have been asked to sign an incident form & been given a copy.
Keep an eye if skin is broken it may become infected & need antibiotics.

What do you need it for? Do you mean to prove it happened at nursery? I'd expect an accident form if it's that bad, but I'm not sure what you'd need it for. Can't hurt to ask if you want it though.

I was just thinking incase it needs to be seen again by the GP. I’m not sure what the normal procedure is and if they would want to see an incident form if it happened at nursery.
I don’t think it will need to be seen as the skin doesn’t appear broken but will keep a close eye on it.

No medical professionals will need to see an incident form. If they were concerned the authorities could contact the nursery and ask them.

The nursery are required to maintain a written record of any accidents and inform parents/carers of accidents their child has. There is no requirement for a parent to sign or receive a copy of the form.

You should definitely have had an incident form to sign.

had the skin been broken by the bite that you took your child to the doctors?

If you want to have a copy of the form for your own records then you're fine to ask the nursery. I don't think you will ever need it for anything though.

Quite common at nursery age unfortunately. If skin unbroken then don’t worry.

We usually ask parents to sign an accident form then it goes into our records. We don't give parents one to take but would photocopy it if they asked.

We are struggling with a persistent biter at the moment but although the bites look nasty on the first day they have usually faded to nothing in a day or two. Keep an eye but should be fine if the skin isn't broken.

When l worked in a nursery, if a child was bitten, we had accident forms. We had two copies. One for our records and the parents had a copy too. That is good practice.
