Present for key worker? Etiquette help!

DS's key worker is leaving as she needs to become a full time carer for her mother which will be a beast of a job.

She's a wonderful lady, clearly very loved by the nursery staff as well as by DS.
Speaking to her briefly she is really sad to be leaving and it's due to the sad circumstances.

I want to get her a leaving present but I don't know what is appropriate.

I was thinking a bottle of wine, some nice chocolates and a card. Is that appropriate?
Should I also get something they can share with the wider nursery staff? Like a box of celebrations?

Help!
(Clearly you can see DS is my first... zero experience).

Not the same but when DC2 moved up from the baby room to pre-school I made a card (one handprint one I did at home, and another moonpig one with photos nursery took type one) with some cash as she'd (his key worker) had been brilliant and we were sorry to lose her (as it were). When DC1 left for school I did chocolate, card & cash. Whatever you feel comfortable with I think, but as she's leaving I'd do a gift specifically for her rather than something to share amongst the other staff. If its something like chocolate she could choose to share it anyway if she wanted.

Given that her finances are likely to change, I'd give her some JLP vouchers.

You could ask one of her colleagues what she likes, unless you already know she likes wine and chocolate.

A card with a lovely message is always appreciated.
personally I love chocolates and wine!
Although you may want to check with her colleagues what she drinks and if she wants to eat chocolate (may be on health kick!)
