I have my assessment next month.
I'm wondering do they tell me that day if I have it or do they have to review my answers.?
Also anything I need to have to hand on the day.
It's been so many years in the waiting I don't want to mess it up.
Thanks in advance.

It should say on your appointment letter if you need to bring anything with you.

If you've had no prior info at all I would assume the first meeting will be a preliminary - it would be a good idea to take a list explaining what it was that made you seek assessment (if your mind tends to go blank in the moment!)

They usually want you to fill in loads of forms and there might be some for family members as well which you'll either email back in or bring to the next appointment.

They don't usually tell you on the first appointment but they might give you an indication. I can't remember if I had two appointments or three now - I know at the one before the "official" one the doctor assessing me said she felt I was a very textbook case, which I still find strange because I don't feel I am textbook at all!
