Diagnosis- questionnaires for parents about childhood

I have heard that part of the diagnostic process in the UK is that you're given questionnaires for a parent (or equivalent) to fill out, about traits you had as a young child.

I'm not living in the UK anymore but have been referred for assessment with the national health service of where I live. After 2 solid hours of questions today, psychiatrist says she can't rule out autism or ADHD, so I will be called back for specific assessments for those.
She has asked that I ask a parent "about some of the things we talked about today" to get an idea of what I was like when I was too young to remember properly.

I realise now I should have asked to write down examples of the sorts of things to ask about, as I just can't remember what was asked in today's assessment.

Does anyone here have the questionnaires they ask parents to fill in about their child? Or know where examples are online? Or knows even roughly the sorts of things I should ask about?

I would be very grateful if anyone is able to help.

It is going to be difficult to ask my mum, as it feels like I'm making light of people who have a "proper" condition and surely it's just that I'm incredibly stupid, lazy and useless at life. But if my difficulties do have a basis, I want to try and learn strategies to cope.

Diagnosed ADHD, waiting for autism assessment.
For autism they asked about my birth so was I natural or C section, any issues? and issues in pregnancy, me as a baby so did I cry a lot, was I fussy, did I enjoy nursery?, first school, pre-school etc. What age did I talk, walk, ride a bike, learn to tie shoelaces and when was I potty trained.

For ADHD, it's basically the ADHD questionnaire as a child so did you have trouble, sitting still, did you have issues playing quietly, did you forget your homework and were you always late for school, was your bedroom messy

Thank you NewYorkCityDreamer. That's helpful and now you say it, I remember that she did ask about most of those today.
