Gestational sac with lots of "tissue" but no pole, high hgc and progesterone

Can anyone help please? Had a scan today at 6w 2d due to spotting all week. They found what looked like a sac but in there words there was no pole, heartbeat or any movement. They said it looked really odd, there looked like lots of tissue in so they gave my a leaflet about pregnancy of unknown location. They took my Bloods.
They've just called to say progesterone is 31 and hgc is high so they need to take blood again in 48 hours. Does this sound like I will just miscarry then? The sonographer seems really confused with the sack. It's not a blighted ovum as didn't look empty ( she showed me)
I haven't had any pain. I'm old tho (44) and have history of early MC but never had this before

Well, at this stage you can't see much (how sure are you of your dates ?)
Also there may be more than one sac, which is perhaps why they mention PUL. As this could be dangerous if ectopic.

What they're doing sounds like the right thing - monitoring closely to see if hcg is rising or if something else is amiss.

Have they mentioned the possibility of a molar pregnancy ? The mention of lots of tissue makes me think of this, but obviously you'd have to ask them at your next appointment.

Just to add - it's not looking straightforward, but if hcg is high you really can't know either way. Do you know what your hcg is ? If very very high it may also be indicative of a molar pregnancy.

Either way it's great that they're keeping a close eye on you.

I did ask about a molar pregnancy but the sonographer said no

Did they say if the tissue was inside or outside the sac ? Or did they mean you had a thick endometer ? (that would be a good sign actually)

Sounds like a potential molar pregnancy to me too (I've had one) but that will become clear with further scans and if your HCG keeps rising. Molars can sound scary but are very treatable.

Inside the sack. Then the nurse said it looked like blood clots inside the egg sack. The sonographer just kept saying how unusual it was. I saw it and it looked like the circle of the sack with grey stuff in it. I've googled molar pregnancy and it didn't look like a bunch of grapes like the images I've seen

Treatment is usually D&C/ERPC then monitoring of HCG levels through blood tests; a small number of women need medication (methotrexate) or further D&C(s) if it is persistent. But it sounds from what you said that it is probably not a molar.

It's easy to say, but try not to worry, don't google too much, and wait for what they find out at your next appointment!
