2 miscarriages - advice on test results/next steps

Hi everyone,

First time poster, long term lurker!

I have experienced 2 early pregnancy losses (first pregnancy twin pregnancy in Dec 2024 at 11 weeks and second a singleton pregnancy in Mar 2025 at 10 weeks) and was after some advice on next steps/what you would do in my shoes.

Medical history of relevant: my BMI is >30 since my first pregnancy, I have hypothyroidism (but levels all in normal range during both pregnancies) and I suffer from ovarian cysts, endometriosis and adenymyosis.

My hospital appointment to discuss my miscarriage is not until Jul 2025, but I’d like to TTC before this date. If there is something I could be proactive about to prevent a third loss based on my blood results below I would be all ears.

A few days before my 10 week scan for my second pregnancy which confirmed silent miscarriage and baby’s heartbeat having stopped at 8.5 weeks, I had my first midwife appointment. All blood results from that appointment are within range except:

Fibrinogen level

  • 4.5 g/L (High)
  • Reference Range:
  • 1.6 g/L - 3.8 g/L

Unfractionated heparin (APTT ratio)

  • 0.8 ratio (Low)
  • Reference Range:
  • 2.0 ratio - 3.0

Vitamin D level, blood

  • 42 nmol/L (Low)
  • Reference Range:
  • 51 nmol/L - 125 nmo

Would you advise I do anything on the above before TTC or once I fall pregnant again? I was taking my folic acid and daily vitamin D supplements.

I don’t know if my midwife would be open to discussing the results with me and my experience with GPs has been poor (but open to your experiences/suggestions).

If I become pregnant for a third time, should I be trying to arrange a midwife appointment earlier than the 8-10 week appointment to get advice/do bloods? Is that even an option?

Finally, a strange thing that happened when I was with the nurse in the EPU after my silent miscarriage was confirmed in my second pregnancy, the nurse asked what blood thinning medication I was on due to me having a previous miscarriage and having experienced spotting during this pregnancy. I said I was prescribed progesterone by the EPU a few weeks prior, but that I was not prescribed anything else. She seemed surprised that my midwife had not prescribed me anything for blood clotting given my previous history of miscarriage, but I advised I’d only had my first midwife appointment a few days prior and when would they have known to prescribe me that? She didn’t say much after that but when I saw the doctor to discuss surgical management she asked again if I was on injections for blood clotting and I said no. This could be because they were booking me in for surgical management the next day and said I would need to have stopped taking that blood clotting medication for 24 hours before surgery, but the first nurse appeared genuinely surprised I wouldn’t have been prescribed them. This may be unrelated, but there was a cyst found inside the sac but it was clear and the notes don’t mention blood.

When Googling the above blood test results that are outside normal range, I see they’re to do with blood clotting so trying to figure out if this is relevant and if I should push for some sort of medication or advice based on my history of pregnancy loss and the results above or as they’re not too outside the normal range am I overthinking?

I’d appreciate your thoughts - thanks so much in advance 🙏🏼

Hello @ThatRoseWasp I am so sorry for your losses. I had my second MMC last week and it’s horrible to be in the position where you just want answers. I know from my own research, that the NHS testing/treatment will only go so far and may not be conclusive, particularly before 3 losses. I’ve taken the decision to go for private testing which I know isn’t something everyone can do, but has a much higher rate of success and quicker answers. Hoping that you get some answers soon and sending a big hug.

@Fletchasketch Firstly, I am so sorry to hear about your losses and hope you are doing as well as can be expected. Sending you lots of positivity and happy and calming thoughts 💕 I think it’s great you’re going private and fingers crossed for you that it gives you answers. I will definitely look into it but was reluctant thinking maybe the second is just bad luck. I think if I were to suffer a third early pregnancy loss, I’d be kicking myself not having the tests done sooner privately which we can afford so perhaps this is the answer and advice I was looking for! So sorry to hear about your losses!
Are you taking the correct dose of folic acid? If your bmi is over 30 you will need 5mg.
Daily vitamin D to be taken with a meal to help increase absorption.
I would speak to doctor about being put on a low dose of aspirin from 10 weeks and progesterone from when you get a positive test.

Firstly I’m so sorry you’re going through this, it’s an awful journey but we all hope worth it in the end!

I’ve had 3 miscarriages and have had some blood tests done where it was found I had factor V Leiden and an elevated APS marker - both blood clotting issues. I would at least ask your GP to have the inherited and acquired thrombophilia screening to rule it out (you might have to push - I did).
I’m pregnant again (very early - 4ish weeks) and have now been prescribed low molecular weight heparin injections and 150mg of asprin daily to help prevent clotting.
Time will tell if it works but I wish I had pushed for the testing after my second as similarly to you, I had a heart beat (and good scan) at 8 weeks nothing on the 10 week scan, with it looking like it stopped around 8 weeks 3 days which according to my consultant, is when the placenta starts to take over.

I was also advised to take 300 u/mg of vitamin d in the form of a spray (can get from boots if in the UK) as my vitamin D is on the low side of normal.

I did end up getting the heparin prescribed by a consultant privately as my GP said it could only be prescribed by a hospital or specialist and my recurring miscarriage referral on the NHS isn’t for several months.

@Pumpkinspice13 Thanks for your message. I am taking the correct folic acid (extra for my bmi) but I’m not taking my vitamin D with a meal so I will do that moving forward, as well as speak to the doctor about the aspirin and progesterone. Thanks again!

@Lalalala77 I’m sorry to hear about your losses but so pleased to hear you are pregnant and have hopefully found the cause of your precious losses and are taking the injections and aspirin. Best wishes in your pregnancy! Sending you lots of positivity 🙌🙌
My second pregnancy did stop at 8 weeks 4 days apparently so thanks for sharing your experience - I will ask for the blood tests and see what they reveal!
