Natural, Medical management or D&C

I would like to hear your experiences of these please.

I have been to EPU today but still undecided on which to go for.

My last miscarriage was at 10 weeks where I had massive blood loss and had to had to have a D&C.

On talking with the nurse who eventually gave me more information than the junior doctor am I unsure, as I was going to have medical management.

I had medical management with mine which worked well but I ended up losing a lot of blood and have retained products do had to have erpc anyway. Did the nurse/ doctor advise given your past blood loss?

Is there an option where you can take the pills and stay at the hospital for them to monitor blood loss?

Starlight39 · 30/12/2019 15:21

Sorry for your loss Flowers.

I had medical management and it was a few hours of cramping and heavy bleeding but nothing too awful. I didn’t have the option of a general anaesthetic d&c though, it was local anaesthetic to the cervix and suction which scared me a bit tbh. I was around 11 weeks pregnant at the time I had the medical management but the baby had died at 8.5 weeks. I left it a couple of weeks to see if a miscarriage would happen naturally.

Hope you come to a decision you feel ok with - it’s such a hard choice to make 😔.

I was given codeine and took that along with paracetamol and ibuprofen every 4 hours. I think the codeine helped me feel a bit less terrified along with lots of Netflix and DP looking after me!

Hey OP! Sorry about your loss!

I had an ERPC for my first MC this year which was relatively straight forward, was in and out of hospital in a matter of hours. Limited bleeding and no pain for the following week or two.

For my second MC in October I wanted to avoid another ERPC due to the risk associated with repeated ERPCs and Ashermans Syndrome. I opted for medical management but had already been steadily bleeding for 1.5 weeks or so anyway.

The night before medical I was at home and suddenly felt very weird, like I was going to faint, then went to the toilet and had really strong cramps. Must've passed passed most of the pregnancy in the next few hours with a lot of serious cramps and pain but once that was over I felt quite relieved.

Scan next day showed medical management wasn't needed anymore. The remainder passed by itself in another 2 weeks of bleeding or so.

Take care of yourself OP, I hope someone is around to give you a physical hug and cuddle as I know how horrible this time is. No decision feels like the right one, just an option amongst options no one ever wants to have to contemplate anyway.

Hope you'll be fine and keep us posted! Thanks

Anny27 the nurse at first kind of said D&C was the better option. Then after seeing the junior doctor I said medical management but he didnt tell me all the info ie: blood loss. The nurse said if I am a natural heavy bleeder to expect alot of blood. The registar then spoke with me but just said either way their is complications. Ie with the d&c they called vac or something said their could be damage to the womb.

Last time at a different hospital they tried the tablets but because I was already bleeding alot explained the D&C a better option. (I didnt stay at same hospital due to treatment before miscarriage which I feel was neglectful).

They didnt mention staying in for longer than 30 mins

Starlight39 the scan said the baby measured at 8 weeks however our last one was 10 weeks. My partner seems to think that because we arent as far on it might not be as bad. But the nurse said that's not the case at all.

I'm just so worried about the loss off blood as by the time we got to hospital last time I had a trail of blood behind me socked through my Jean's it was horrible and embarrassing. Being stood in the car park. I think if anything it is the blood loss that is worrying me.

@LASandOtto hello I didnt know about that syndrome thank you for sharing that. My partner is with me. He is totally amazing, yes it's very hard either way I choose.

I had a D and C at 9 weeks.
Was home after a few hours to begin grieving/healing.
I wanted everything finished as soon as possible.
Take care.

Guest1233 · 30/12/2019 16:54

I think I've decided on a D&C nurse questioned me when I rang back. But I do feel it's the better option. Given last time I ened up not sleeping because I thought I was going to die after the blood loss and having panic attacks (never had them before). Its horrible isnt it, not only do we lose our babies we have these big choices to make.
It wont be till Thursday Friday and may go naturally by then.

Thank you so much for your support ladies it really does mean alot.

Hi @guest1233 I remember being in your shoes this time last month and it was awful. I was so torn by what to do for the best as none felt like the right decision. My initial reaction was I didn't want surgery and wanted the medication but after some more discussion I changed my mind to surgery. I too was worried about blood loss and how horrific the experience could potentially be. I also felt if the end result would be that I needed surgery anyway I might as well just get it done out of the way. However, I was also really worried about the potential complications with the surgery - especially ashermans syndrome as mentioned above. This was dismissed by the nurses I spoke to as they had never heard of it so told me it must be really rare! The nurses were definitely more pro-surgery and felt this carried less risks. I got to speak with the consultant on the morning of the surgery who reassured me that in her opinion ashermans is very rare and she had only seen two cases from doing these procedures daily for 5 years but obviously it is still a risk and the pp is right that it's more closely linked to repeated surgeries (especially after child birth though). From what I've read about scarring (and it's a lot 😂) it can usually be successfully treated if caught quickly so keep an eye out for any symptoms which may suggest ashermans or scarring post surgery.

Guest1233 · 30/12/2019 20:16

@GoldenBauble what are the symptoms for ashermans or scarring? Can it be picked up on transvaginal ultrasound?

OP posts:
GoldenBauble · 30/12/2019 21:25

@Guest1233 from my understanding the main symptoms are loss of period or very very light periods (different to what you experienced before) but you will still get the monthly symptoms and the cramps can be particularly bad as the blood is unable to leave the uterus. It can also cause infertility as it prevents the egg from attaching to the lining so if you got pregnant easily before and then struggle to conceive this may also be a sign. The scarring is not usually picked up on an ultrasound (although I believe there are a few consultants who can now detect this with the very advance 3D scanning machines which aren't used in the nhs) so generally diagnosis comes from them using a camera to see inside (can't recall what the name is, Hysteroscopy maybe). The good news is that generally the scar tissue can be removed if it isn't too severe and I believe the prognosis is generally pretty good (something like 75-80% of treated women go on to have successful pregnancies). If you search on here there are a few old boards with ladies who have had personal experience. They're aren't many people who seem to post about this though so I also use that as reassurance that it must be fairly uncommon otherwise more women would be posting. Hope that helps a little, I got myself really worked up about it and have done hours of reading.

Megabustravelling · 30/12/2019 21:37

I had an erpc earlier this year and it was the right decision for me. I bled for two weeks after but hardly any pain.

Guest1233 · 30/12/2019 21:57

@GoldenBauble thank you I did have a read up on it in the Internet. I will know if I ever have any symptoms as my periods are heavy with minimal pain. But it is always good to know about possible changes so I can keep an eye on it

OP posts:
Guest1233 · 30/12/2019 21:58

@Megabustravelling yes I feel it is right for us. My partner is just as worried as me about the blood loss we had with our previous miscarriage.

I'm so sorry for your loss OP.

Is MVA an option for you? It's a procedure with a local anaesthetic. I had one on Christmas eve and I can assure you it was absolutely fine. Over in 5 minutes. I had a bit of cramping and period like bleeding for the rest of the day, then light bleeding, and today is the first day with no bleeding. I would absolutely go for this procedure again if I was unfortunate enough to be in the same position. If you would like more info, please ask. Whatever you choose, you will be OK Flowers

Emanning · 30/12/2019 22:28

I had a surgical miscarriage 10 days ago, it was successful, and had hardly any bleeding or cramps to start with but now I am having bouts of bad cramps followed by passing of large clots is this normal?

GoldenBauble · 30/12/2019 23:00

@Emanning I would ring your epu for advice. I had no pain at all following my surgery. The bleeding was a bit intermittent, it would stop and then come back worse than before but eventually it tailed off. I never had any clots (apart from one really small one) and I was advised if I experienced any clotting, pain or heavy bleeding to contact epu. Hopefully it's nothing to worry about but you're better getting some reassurances

Guest1233 · 01/01/2020 10:46

@Emanning Hello hun did you EPU to get some advice?

OP posts:
Emanning · 01/01/2020 21:07

@Guest1233 sorry for my late reply and thank you for checking in. I did go and get advice, I ended up in my local hospital where I had some further scans, it's turns out my surgical D&C wasn't a success so I have now taken the pill form to try and expell the rest myself, I took the pills yesterday lunch time and so far nothing has happen but I guess I just need to be patient...i was given the choice of the pills or more surgery, but truthfully iv lost a bit of faith in the surgeon so fingers crossed the pills work.
