Hello, I recently had an internal ultra sound scan after reporting some irregular bleeding afterwards I was sent to an appointment with the gynaecology team I was told to come off Desogestrel as in his words I don't need it, I'm 53 so I was hoping to carry on taking it till 55 and he upped my progesterone dose to 200mg as he said as I am taking 3 pumps of oestrogel I need a higher dose of progesterone this is on continuous he said any more bleeding in the next 6 weeks I should let them know but won't I have a withdrawal bleed from stopping the mini pill anyway so wondered if anyone else has had any similar issues thanks

The guidance is to consider 200mgs Utrogestan with 3 pumps of gel, depending on whether you have had breakthrough bleeding.

HRT which is continuous will control your womb lining, so it's not the mini pill that does that.

It's well accepted that at 53 you probably don't need contraception.
The guidance is to carry on with contraception for 12 months after a final period after age 50. Obviously you don't know if you're post menopause because you're using the mini pill. But the chance of conceiving at 53 and the pregnancy continuing is very remote.

All of this should have been explained to you at the time.

Thank you very much, what you have explained makes sense but like you have mentioned none of it was explained to me at the time although maybe I should have asked more questions myself, I was just so relieved to be just having a conversation about changing my hrt medication as I had been told by the reception staff over the phone that I needed a biopsy so I was petrified and confused when I got there and like I said ended up just being asked questions and changing medication

It's easy to get sidetracked in appts!

Hopefully you feel a bit better now.

I've actually started spotting today, is this a withdrawal bleed from coming off the mini pill which was last Tuesday and maybe also change of dosage I don't want to inform the gynecologist if it's just a side effect

Probably. To be honest, you weren't advised very well. You should have been told that you might have spotting etc (or not ).

If your scan was normal with no thickened areas it's probably just timing to get the dose right with the HRT. They don't redo a scan so soon again anyway, so I'm not sure why you had to report any more bleeding.

Did you get the results of the scan and what does it say? (If not, you should so ask your GP for the results.)
It should say the lining was Xmm thick, normal etc.

It's bound to take a while for the higher dose of Utrogestan to take effect.

I had a mirena fitted in January for heavy unpredictable bleeding even on provera and they said your follow up is on May 3rd or whatever. Got an email to say it was cancelled. Nothing since! 🙄 luckily I have researched myself and things have mostly settled now but still 😂

Joanne73460 · 24/06/2026 14:48

The only feedback I got about my scan was that they couldn't get any measurements from my womb lining as it was in the wrong position I'm assuming the scan device was what was meant surely not my womb and if I had anymore bleeding they would go down the route of either a camera inside or another scan I feel like my GP would be better at explaining things but they usually say if you are under a specialist you need to ask them

Just checked my online GP service the letter the gynecologist sent to my GP does mention measurements couldn't be taken due to my uterus being positioned wrong so no explanation of that to me is this a cause for concern or something they may look at again

That is not acceptable.
Anyone doing a scan should be able to get measurements if they are qualified enough. I think I'd ask your GP to refer you to a consultant who can do the scan again.

I'm due to have a cervical smear test next Tuesday at my own GP surgery providing I'm not bleeding I'll try asking if I can get some more information and advice
