Bleeding after starting HRT gel with Mirena coil, is this normal?

Had mirena coil fitted recently no issues.
Was given hrt via gel pump which has been absolutley amazing.
Ive now randomly started bleeding last 3 days and swollen tummy.
Ive had mirena years ago with no bleeding.
Really comfused and dissapointed. Ive checked my strings and can still feel them.
Ive now stopped my hrt pump to try stop the bleeding which is a big shame.
Is this normal please?

I had this happen and it was caused by fibroids so id suggest a conversation with your GP.

Similar to you in terms of gel and mirena.

I’ve had a little spotting (not fresh blood), but my biggest issue is period pain like cramping. The meno nurse told me it can take up to 6 months for the Mirena to settle, but having not had period pain for 20 odd years on Cerazatte (mini pill), it’s hugely frustrating.

Go back to your meno nurse or GP if you can, to at least get some assurance if nothing else.

The thing that confuses me is that years ago i had mirena coil.no bleeding now this. Im wondering if its a hrt issue which unfortunatley why ive stopped with hrt with hope it stops the bleeding

In my case the estrogen fed the fibroids which caused them to bleed. They weren't an issue before I increase my gel to two pumps.

Thankyou. Actually this did happen after increasing to twp pumps.
What did they do to help you

If its a new Mirena they can cause bloating.

I still had periods with my mirena on and off for 18 months. And horrific period cramps.

There are different options depending on how much of an impact the fibroids are having (if that's the cause for you). I had a lot of them and some other complications so ended up with a hysterectomy (which has been brilliant for me). Your GP or nurse would be the best to give you advice

@Merseymum1980 I'd try not to worry.
With continuous combined HRT, there can be spotting or bleeding for up to 6 months. The progesterone in the coil does not work instantly and neither does it in patches or tablets.

You may get a different bleeding pattern as it's part of HRT.

The guidance is to allow at least 3 months and 6 months for any continuous regime to settle. It's not likely there is a problem but do ask your GP.

https://www.menopausematters.co.uk/postmeno.php

"Period free" or continuous combined therapy can be used by women who are 54 + yrs, or more than one year since last period at any age. The criteria should be fulfilled in order to offer such treatment to women who no longer have a continuing ovarian cycle, so that steady levels of both estrogen and progestogen can be achieved. When there are steady levels of estrogen and progestogen from daily administration of both, the womb lining stays thin. Although some bleeding in the first 6 months of therapy is common, there should not be bleeding after that and the lining does not go through the stages of stimulation and then shedding as it does during a normal cycle and with sequential therapy.

TrashHeap · 31/03/2026 22:24

You need to give it time to settle in

I went to gp yesterday. The coil has not moved or anything.
Im still cramping and bleeding. I have to reduce to one pump instead of two.
Hrt has been a miracle for my mental health but im really considering stopping it because of this bleeding and cramping.
I felt like gp was hinting i didnd actually need it as im perimenopause and my ovaries are still producing osterogeon however it really helped my extreme anxiety id started to experience.
Has anyone any experience please
