Cyclical HRT: periods and ovulation symptoms returning to 23 day cycle?

Been on cyclical HRT nearly a year. Mid 40s.

At the point of starting HRT, my period cycle had fairly quickly changed from the 23 days it had always been, to irregular, much longer cycles.

HRT worked amazingly well. I start the ultrogestan on the same date each month, so a new roughly 31 day cycle. I upped the estrogen patch strength about 6 months ago to 50 when my migraine crept back, but otherwise no issues. Until....

Is this even possible? For the past 3 months I feel as though my old natural cycle has returned. I've been getting ovulation pain, then a few days later my libido disappears, I start getting sore boobs, I get tired early and feel unadventurous, sometimes feel bloated. And then (after a day or two of intense PMT) a very normal period appears on day 24.

The first time this happened was near the start of the ultrogestan part of the cycle, but I diligently carried on taking it throughout, and after, my period. Last month these feelings of making my own progesterone were just so strong I didn't dare take the ultrogestan on top because I think I would not have been able to function during the day.

Has this happened to anyone else? Does this mean I need more estrogen again?

(Obviously I'm asking a Dr too but I don't expect to hear from them for a few days.)

It's possible for your own periods to override an HRT cycle.
If this happens you just carry on with the normal HRT cycle until it settles back down.
You're more likely to experience this as you're very young to be on HRT (not unusually so but a lot of women don't even start till early 50s) so your own hormones will still be very powerful. Some months you will ovulate and some months not.

Your GP is likely to say just stick to the HRT cycle.
If you feel you need more estrogen, you can do that as an experiment.

Thanks for your response! That makes a lot of sense.

I think my generation is really lucky to have had so much more info on HRT. Most of my female friends of a similar age are on it or considering going on it.
