Does vaginal estrogen help sexual function?

I'm 53 and I've been on HRT (estrogen gel and progesterone) for almost 4 years. All good.
However, im noticing that in the last 6-12 months it's taking me longer to orgasm during sex. And my orgasms are definitely weaker. Everything else is similar. Im not experiencing dryness or discomfort.
Has anyone experienced an improvement in sexual pleasure by using vaginal estrogen?

I have been medically menopaused and cannot take hrt any more. I take local oestrogen and I am seeing a women’s health physio.

Are you doing pelvic floor exercises? That will definitely strengthen up muscles and bring your old self back. There’s lots of strengthener products if you wanted to have some resistance and/or feedback. I have gone for it with a Perifit and it has been amazing. You essentially play games on your phone, connected to the device 10 mins a day 5 days a week, by week 6 I noticed a very real difference in this dept. My women’s health physio recommended it and specifically the lilac one. I am on vaginal oestrogen too and have been for months, it helps with other symptoms hugely but the exercise has made the biggest difference by far. They aren’t cheap but I do have a referral code if you want one, pm me if you do! It costs as much as two physio sessions but you can use it for years, good value in my view. Good luck!

I’m on it and it hadn’t helped with this. @wateraddict is the strengthening for vaginal area or cliteral area if that is not TMI. Asking as I believe atrophy of the latter is a large factor in this issue and I had not heard of women’s health physio as an option.

Ah! A good contraction works the whole pelvic floor so I think the exercises are worth a go, especially given so much of the clitoris is under the skin, but I would definitely speak to your GP and give local oestrogen a try. I really hope it improves for you x

I found testosterone helped with this…

Betty91 · 24/08/2025 12:27

I got topical oestrogen for this and can't get along with it after a few starts & stops - I tried pessaries too. It seems I'm the 1 / 100 who gets side effects including sickness / headaches / burning / UTI etc.

Wonder if I might think about testosterone and Perifit .... as not sure I can do HRT either as progesterone intolerant too.

Thanks for that @wateraddict I'd known about pelvic floor exercises but forgotten it. I've got special weights I haven't used since having my 2nd child. I think I'll start using them again 🙏
@Delatron That's definitely an option
Thank you

Great that you have something to go with right away, @AreWeThereYet69i have all crossed for you!

@Betty91happy to send a referral on pm if you definitely want one (they have given me 10x discount codes to pass on)

LittleJustice · 28/08/2025 00:17

@wateraddict please could I have a referral code for the perifit. I have pmed you.

Also can anyone explain how to access testosterone to see if that also helps.

Delatron · 28/08/2025 07:08

Some GPs will prescribe testosterone these days. It’s licensed for low libido so you have to say that. I went private though as my situation was complicated and I wanted more expert advice.

I’ve just been to a gynae for sporting. Turns out that even on HRT you can get vaginal atrophy and dryness. Systemic HRT doesn’t do a great job of keeping the vagina and vulva happy so I’m imagining clitoral function also. I’m now also on pessaries for inside and cream for outside
