It's possibly because you're on a higher dose of gel.
Utrogestan 100mgs is sometimes not enough to control the thickness of the womb lining on higher doses of estrogen. You could be using 200mgs a day.

If you're not post menopausal, your own estrogen could still be circulating too, which means you can also get spotting.

Usually, if you're under 50 when you start HRT and not sure if you are post meno, it's better to start on sequential HRT ( a cycle like your own periods) so you have a regular withdrawal bleed each month. This avoids the spotting etc you can get it if you start continuous too soon.

You say you've not had a period for years- if that's the mini pill, it could be masking your own hormones meaning you are not post menopausal.

It won't be serious in all probability.
Talk to your GP and ask to increase your Utrogestan to 200mgs a day.
This is medical guidance from the British Menopause Society produced for GPs, so yours should be able to read it- it's all online anyway for anyone to read.
3 pumps is considered higher so it's quite possible that all you need is a bit more progesterone.
Your GP might refer you for a scan (ultrasound.
Don't worry.
