Clitoral atrophy, no orgasms to speak of already on HRT and Test

I'm 54 have been on HRT for a few years and testosterone for about 18 months (the usual doses, about 3 pumps of Oestrogel dialy, daily progesterone, and a sachel of testogel lasting 7-8 days)
Have been on antidepressants for decades whcih doesn't help i know but the decline in my clitoris has really happened in the last 3 years or so - i can barely have more than a dribble of an orgasm even if aroused.
i wish I'd mentioned it to the GP years ago I'm just very embarrased by these things.
I've done a lot of reading and am feeling disheartened as most people seem to think testosterone will be the saviour wheras it hasn't helped me at all. I'm prepared to try Ovestin although women with bad atrophy don't seem to find it helps much. I've read about toptical sildenafil 3.6% but I don't think it's available in the UK.
Has anybody seen a good specialist about this? I would be prepared to pay to get my sex life back!!

I know you say you're embarrassed to see your gp but honestly, it's their job to help us with our health and wellbeing. They've heard it all before and aren't remotely bothered so I'd go back and see what they suggest. If you've been on testosterone 18months have you ever had bloods checked, do you know if in therapeutic range? You may just not be absorbing it so doses could be tweaked.

Always worth trying oestrogen cream directly to the clitoral/vulval area.
