Utrogestan pessary and burning

I'm hazarding a guess that the recent arrival of bright red and sore bits might be down to vaginal utrogestan? I use vagifem twice weekly and have recently started 200mg pessary every other day plus lenzetto 1 spray (as per consultant). Previous Mirena insertion was horrible - think a bloated Little Miss Angry with constant head pain!

I'm trying this regime as the 14 days sequi regime was awful after day 3 of prog. I'm on a low dose of oestrogen due to disordered proliferative endometrium though Ive had no period now for 3 months. Prior was regular. Side effects at present other than the soreness are hot flushes and night sweats (not had them before so thats an unwanted gift!), early morning waking with a bit of anxiety, lower abdo pain and bunged up. Benefit is I'm slightly less achy which was the main reason for trying it.

My consultant says it's this or nothing. Right now my fluff is voting nothing! Might it ease?

Unfortunately it does seem to be a side effect of using utrogestan that way.
(I experience it too along with needing to wee more, although I'm using it on a cycle.)

I can understand your dr being cautious, but it would seem that you are having such a low dose of estrogen that it's barely helping.

Was there a reason for your thickened lining? Was this atypical hyperplasia? Did the link it to HRT or were you simply in peri?

Would you or your dr be willing to consider using Utrogestan on a cycle, or 100mgs daily rather than 200mgs on alternate days? (I don't understand the logic of every other day, to be honest.)

Hyperplasia was simple non atypical after a few weeks of irregular bleeding. They also removed a polyp. The bleeding stopped and two further biopsies over a year showed mild proliferative endometrium. I now haven't had a period for three months. I tried the 14 day cycle but couldn't get past day 3 with extreme drowsiness in the morning meaning I struggled to drive and very low mood.

Just spoken to the Dr who has told me to keep taking the utrogestan but stop the vagifem I was using alongside to see which it is.

That's interesting. There is so much that could be discussed here, but it would take up your time. For example (this is based partly on my own experience) I know that the timing of a scan can be crucial. If it's done at any time in the cycle, then the linking may be thick(er) especially in peri. The scan has to be done immediately after a bleed to get a true result.

If you have had simple hyperplasia, why is that a concern if you were not post menopause?

A slightly thicker lining is not usually a concern during peri as it varies so much.

Are you working on this with a specialist menopause gynaecologist?

@JinglingSpringbells yes a consultant gynaecologist for the hyperplasia now on annual review and a meno consultant. The reason I was fast tracked was simply my age. He kept saying I was told old to be having periods.

How old were you? Up to 55/56 is acceptable.

It comes over as a sledge hammer to crack a nut but I'd hate to go against 2 experts as I don't know the whole story.

You're on a really low dose of spray and using a large dose of Utrogestan.

Are they not happy for you to use the normal dose of gel or spray, 100mgs Utrogestan daily, and annual scans? What's the reason for alternate days for Utrogestan?

That's exactly what I said re too much utrogestan. I asked about 100mg daily and it was a no. I'm so fed up with it all. What you say seems perfectly sensible

Could you get a 2nd opinion?

Have you access to a private menopause specialist?

Could you afford a one-off appt?

From what you have said, it isn't clear that the hyperplasia was anything abnormal or of great concern (I'm not sure if your periods had stopped at that point?)
