Bleeding - HRT was great for 6 months

I have been on Evorel patches and Utrogestan for just over 6 months. It has been going pretty well, dose was tweeked from 50 to 75 a couple of months ago. I had been having a regular bleed every month.

This month, 2 weeks after the expected bleed I am bleeding again (has been 5 days so far). Obviously I am going to see GP about it but I wondered if anyone has had similar. I'm trying to work out what's going on. I have felt like I have had more of the peri symptoms that I had before HRT and have had quite a few gastric issues - maybe IBS. I've been a hormonal wreck this week and just hoping it can be resolved!

And I didn't say she shouldn't have a scan. I said it may not be 'useful'.
It may be if it's timed correctly, but that's an issue in itself.

@WitchyFingers2 The BMS now suggests increasing the Utrogestan to 300mgs for women on 100mcg patch. Maybe your GP didn't know that? The outcome you had was possibly due to that under-dosing - which results in a thick lining.

I'm grateful for all input folks! I am always aware of the warning that MN puts up before health threads and would never blindly follow a poster's advice. I think the main problems is that menopause related issues are not an exact science so it often feels like we are bumbling along in the unknown. So it's good to get an idea of different experiences.

OP I went on oestrogel (one pump) and utrogestran for 12 days last January. For first 8 months I just had periods roughly every 28 days but then had a medium flow bleed for 24 hours in August in the middle of my cycle and the same again last month. I have a GP appointment in 2 weeks so will report back if useful..

Christine0708 · 04/11/2023 11:10

Out of interest how were you taking your utrogestan? Where you staaetingbit on the same date each month? X

carkerpartridge · 04/11/2023 12:11

Christine0708 I was taking it for 12 days a month. I wasn't taking it on the same day every month...the fact that you and others have mentioned this makes me think that I wasn't taking it correctly!
When I was prescribed HRT the doctor (over the phone) told me to change the oestrogen patches twice a week, which is what I did. She also said to take the progesterone (Utrogestan) for 12 days starting on day 15. So from the beginning of my taking HRT I took it that my "cycles" were 26 days...this seemed odd at the time but seemed to be what the doctor advised. So I really don't know if I have completely misunderstood how to take the Utrogestan.

An update in case anyone is interested!

I saw a lovely GP who referred me on the 2WW due to the unexplained bleeding. She actually said i could carry on with HRT if I wanted but I decided to have a break until the investigations had been carried out.

I had a hospital appointment 10 days after GP appointment. Had a transvaginal scan (which showed womb thickening just over the recommended level and a simple ovarian cyst) , smear test, blood test (for ovarian cancer) and biopsy of womb (due to the thickening). It was a worrying time but also good that a lot of checks were done and quickly. I was told that it probably was my own hormones creating a bleed rather than anything concerning. The concern seems to be that since I have had periods within the past year it is unclear whether I am menopausal or not, so the medical advice is to take any bleeding seriously.

Anyway I had my results yesterday (quicker than expected which was good for my stress levels!) The biopsy and smear have come back clear which is a huge relief. I need to have regular scans to measure the ovarian cyst to make sure it doesn't grow. This is apparently since it could be of concern if if I have gone through the menopause. So fingers crossed that it behaves!

I will have a chat with GP about what to about resuming HRT. I felt dreadful after stopping it for about a week but have felt pretty good since then so I don't currently feel desperate to go back on it!!
