Bleeding on HRT?

marshmallowfinder · 26/09/2023 13:01

I am on daily Oestrogel and Utrogestan on days 15-26 of cycle. Plus occasional intra-vaginal oestrogen pessary. The period like bleeding that occurs after day 26 every month is getting me down so much. It's quite heavy, flooding at times and I can hardly cope any more. After 40 years of periods, I am so done with it. Is the only way to stop it to come off HRT completely, in the hope that I'm finally post menopausal? I'm really unclear about this bleeding in general. I'd be so grateful if anyone can help. Thank you.😔

How old are you OP? How long have you been on HRT? We're you still having periods when you started it?

Sorry should be were

Hi, I'm 52 and was having periods, yes. I've been on it 1.5 years. Am I right in saying that the monthly bleed is necessary to prevent the womb lining building up and it's not actually a period?

Yes the bleeding is a response to the progesterone which prevents too much lining build up. At 53 you could try a change to continuous combined HRT with which you shouldn't have bleeds. However if the bleeding doesn't settle then you should definitely discuss with the Doctor whether you require a hysteroscopy to check the lining is OK.

Thanks for your help. It's a few months before I return to the doctor, but I'll bear it in mind for then. 🙂

I would go back sooner if you're flooding.

As you are around the average age of menopause (51 and you’re 52), and as you’ve been on a sequential regimen for
1.5 years, it’s appropriate to ask for a continuous regimen, ie. Daily progesterone. Any change of hrt can cause bleeding for 4-6 months whilst settling but hopefully you won’t bleed. The progestogen element can be tweaked, search for progesterone on the British Menopause society website, if a woman still bleeds after that amount of time, she may need a synthetic type of progestin such as Provera but Utrogestan is plant based and has a lower risk profile and could be doubled for annoying bleeding. Your GP needs to discuss this and send you for checks if they think there’s a clinical reason ongoing and they want reassurance.

Why don't you just change yourself to taking 1 utrogestan every day instead of 2 for 14 days. I did and nothing bad happened, quite the opposite
