Lenzetto and 100 mcg patches

Hi all. Just wanted to know if anyone has done this. I am awaiting my GP to call next week to confirm this plan. Currently on 100mcg everol patches with 200mg utrogestan 12 days a month. I have had a huge improvement in symptoms but have never had my night sweats controlled.
After tests to investigate other causes of night sweats my GP suggested I may not be absorbing the patches well and as I am 45 its possible I just need more oestrogen.

So, the plan is to continue on 100 of everol with the utrogestan but add in lenzetto. My GP wants to clarify the safety of this as high doses of oestrogen are linked to endometrial cancer, so I am awaiting a call to confirm the dose of utrogestan. I thought 200 was the max for progesterone? I am a bit concerned and just wondered if I am better dropping the patches altogether and sticking with the spray and utrogestan. I can't find a great deal online either way.

Has anybody done this?

Just me then! I think I will stop the patches and stick to spray and oral progesterone. Will run it by GP but scared to take too much oestrogen.

Thee is guidance by the BMS on this.

Over 100mcg patch (or 4 pumps of gel) they advise 300mgs Utrogestan.

Ah thank you @JinglingSpringbells I will take a look. It just seems like a lot to take and remember, as the suggestion is 100 patch twice a week 1 spray of lenzetto daily, 12 days of utrogestan and twice weekly vagifem.

@Battlecat98 I wonder why your GP didn't suggest swapping to estrogen gel? At least with it you can increase it easily without adding in a 2nd product.

I'd say you should try gel and start with the max 4 pumps and see how that goes.

4 pumps is the same as 100mcg patch but you might absorb it better. And you can stick with 200mgs Utrogestan at that dose.

Be warned, 3 sprays of lenzetto only equates to a 50mg patch. One spray is next to nothing!

You may be better off trying the gel first. Although 4 pumps is alot of gel to slather round the body.

As I understand,100 utrogestan is fine for up to a 100 patch / 4 pumps gel/ 6 sprays. Not sure about oral oestrogen.

After the licensed maximum dose, you would be under a special anyway.

100mgs is for women using it daily (usually after their own periods have stopped for good.)

200mgs is for women using it 12 days a month (either in peri or who chose to carry on with a cyclical regime.)

I know, I am still on cyclical so Utrogestan for 12 days a month and because im not enjoying the effects of the utrogestan that much, at my 6m checkup my GP said it should be fine to only take 100mg of Utrogestan for the 12 days i.e 1 tablet daily because I am only using the lowest dose patches - 2 x 25 mcg Evorel per week, which I am fine with, controlling symptoms fine.

I'm just wondering if anyone else has heard of that or if it is official guidance for lower does of Estrogen that it's ok to only take 100mg of Utrogestan nightly for the 12 days?

It's not official guidance, no. The doses are licensed to use with the estrogen so any variation should be under guidance. (I use an off-licence amount but this is privately, under a consultant, and I have a private scan every 12-18 months.)

25mcg patch is the same dose as 1 pump of gel.
It's possibly ok but you should maybe ask your dr again and also ask about monitoring your lining.

Everyone reacts differently. I bleed a lot on the cycle, whereas I know someone who is using the same estrogen dose as me (in fact slightly higher) and the same dose of Utrogestan and they barely bleed at all.

Thanks. I had a private scan before starting and am planning another at the one year mark so hopefully any issues with this would show up then. GPs have always been pretty useless on HRT and you never see the same one twice.

Only think I've noticed since the 3 cycles of lower Utrogestan is that I bleed slightly earlier in the 28 day cycle, not sure of the sgnificance of that.

Do you have a link to this guidance please? I take 100mg of utrogestan daily without a break and use the evorel conti 100 patch (to control my mood, I'm47 and as far as i know, still in peri) just want to make sure it's enough utrogestan as I still bleed a lot. I have a gp appointment next week to discuss this but have a feeling the gp doesn't have a clue unfortunately 🙄

Oh it's OK I've found it, apparently 200mg per day should be considered. Not sure if the gp will prescribe that though with all of the utrogestan shortages 🤔

Is this a name change fail as I can't see you Q on the thread? @Meerkitkat

The guidance is

100mg daily continuous combined regime
200mgs x 12 days per month on a sequential regime

More than 100mcg patch or over 4 pumps gel- consider 300mgs progesterone daily on sequential regime.

Or 200mcg daily on continuous combined.

Not sure how your GP can disagree with guidance.

The BMs does say that the licensed doses of estrogen should be enough anyway and women ought not to need more than what is the equivalent of 3mgs estrogen a day.

Thank you for the guidance, the GP I tend to speak to is the 'menopause specialist ' but this time it was a different one, she seemed a bit confused about the gel/spray and asked if I wanted the patch as well as a spray? I mean I really have no idea. I just thought I would switch.
I feel confident going forward. I will ask to have the gel and utrogestan only.

So just to update my GP still has no answer regarding my utrogestan so... Has suggested I start provera instead for 14 days of the month. Honestly I think I will just put up with the night sweats as my HRT now consists of, 100mcg everol and vagifem twice a week, daily lenzetto and provera 14 days a month.
I really don't want to change to provera tbh I am fed up with it all.
My symptoms were pretty well controlled otherwise just the night sweats. Who would be a woman?
