Directions on packaging very unclear, I’m still menopausal.

Did your GP not explain? Is this your first HRT or are you changing from something else?

The patches are usually 2 per week, so if you start tomorrow, you'd change it on Tuesday and then again Saturday - the box should have a "reminder" of which days to change.

The utrogestan is usually 2 x pills daily on days 15-27. Count day 1 as the 1st day of your bleed. But after that, you follow a 28 day regime regardless of if/when you bleed. Your body should follow your regime, not the other way round (I got this wrong for 2 years, could have had serious consequences).

Check with your GP though, as instructions can vary.

G/p did mention pill form, but chemist failed to mention that the pills were out of stock so just assumed it was patches only.
1 month of everol patches… felt ill.
so I did the correct regime this month, but no period?
Trying to take the pill 2 hrs after eating at night is impossible as I’m up at 5 am for work, would be better for me to take in am, even with contra indications re sedative effects?
i find safety net for medications is very poor.

Bit confused by your post in parts :)

You said you felt ill on the estrogen-only in the first month.
This is nothing to do with not using Utrogestan. You have been started on a very high (highest) dose of Evorel. Most women try a 50mcg patch to start.
Why the high dose?

What time do you eat your dinner at night?
If you are up at 5am, are you eating early at night (like 6pm) and going to bed around 9pm? You can take it as you get into bed.

You can take it on a full stomach- all that does it make it absorbed twice as much as without food- which is not necessarily a bad thing, it might just (for some women) increase side effects. (It didn't for me - no difference with or without food.)

One main side effect is dizziness so not a good idea to take in the morning if you are off to work.

Not all women bleed the first month on HRT- see how it goes.

You must take your utrogestan. I know there's a shortage (I've got 1 month left!) but if it happens again, contact your GP for an alternative.

It's best to leave a gap after eating, but if two hours isn't possible then just take before you go to bed. Mornings are not advisable - it will likely make you drowsy.

If you don't get on with the pills, ask your GP for Evorel Sequi - this is two weeks of Evorel patches then two weeks of Evorel Conti which have estrogen and progesterone combined. It's a lower dose estrogen 50mcg, so you'll need to top up probably with additional Evorel patch.

100mcg estrogen is the max dose! Normally you start on 50mcg for 3 months and see if your symptoms ease, and increase by 25mcg if not. When you say you're feeling ill, do you mean a constant nausea? That would be the high estrogen - it's like the flooding you get in the early weeks of pregnancy. It should ease in a couple of months as your body gets used to it.

I don't get a bleed every month, and the fact you skipped the progesterone may be a factor.

You should have a review after 3 months, best to make that appointment for 2.5 months after you started so if you do change regime you've got time to get the prescription filled.
