Have I left it too long to get HRT

I'm at wits end! The rage! The bloody rage! I'm unreasonable and I know it.
The detail: I'm 55 and been in peri menopause for 5 years. Periods have slowed to one every 6 months and when they arrive it is like the chainsaw massacre. ( saying that last period was in January, and some spotting in August).
Main symptoms:
Night sweats - constant, from the minute I lay down in bed. My hands hurt, my body glows and drips with sweat.
Dry vag ( it's like the Sahara) and complete loss of libido
Brain fog - I had no idea how to drive the car yesterday
The rage - this is relatively new. I'm just so cross all the time and "fuck" or derivatives of it is my most frequently used word.
So, I was trying to get through without HRT but how much longer will this go on? Should I try HRT?
My main questions are will it make periods return? Will I put on weight and get hormonal migraine ( I did with the pill)? And, if I take it for a few years will the symptoms return when I stop?
Sorry this is so long, I'm struggling now and dreading another disturbed night.
Any advice gratefully received.

@ProperVexed oh your poor thing, that sounds horrendous. OK, my reply will be long, so buckle up.

Like you, I was trying to get through the menopause years without HRT, but I finally cracked earlier this year at the age of 58 and after a lot of discussion my GP agreed to prescribe. I'd thought I was doing OK, but I was curious about HRT and whether it would get rid of my annoying symptoms, and my goodness what a difference since I started, I'm like a new woman.

Given your symptoms, I think you should give HRT a go. Also, you might need Ovestin for the dry vagina - it'll make a big difference.

My GP put me on Oestrogel, up to two pumps per day depending on how I feel, plus a daily Utrogestan tablet for the progesterone component. Progesterone is used to protect the womb lining, some women have a Mirena instead, but I had mine out a few years ago. I haven't had a period since I got my Mirena in the midst of peri aged 46, so that's why I take daily progesterone. Women still having periods take Utrogestan for just part of their cycle.

I had one bleed since starting HRT six months ago and I won't lie, it was like a horrendous painful period, but since then nothing. I haven't gained weight and haven't had any headaches. I sleep well, night sweats have vanished, l don't ache everywhere any more, I have a comfortable undercarriage again and my libido has returned. My brain fog isn't too bad, though in my late 40s it was horrendous, and I've never really suffered from rages but I feel on a much more even emotional keel.

Have a chat to your GP, but do your research first. Go in knowing the risks and benefits, and if you can tell the GP what you think you need it'll help - some don't have much of a clue, unfortunately.

Best of luck!

God replies there from @DramaAlpaca So, I was trying to get through without HRT but how much longer will this go on? Should I try HRT?

The only way to know how long is with hindsight. Some women are lie this forever without HRT. Seriously. About 10% have symptoms for life.

My main questions are will it make periods return?

No, if you use a type of HRT that doesn't cause a withdrawal bleed.

Will I put on weight and get hormonal migraine ( I did with the pill)?

No reason to put on weight (but we all need to eat less as we get older.)
I've used it for over 12 years and weight the same as before (low BMI.)

HRT is not like the Pill. It's a different type of estrogen.

And, if I take it for a few years will the symptoms return when I stop?

You can choose how long to stay on it. If you stop and symptoms are still there, you can take it forever.

PantyMcPantFace · 24/10/2022 14:09

I put on weight with the combined pill. I was suicidal/murderous on the POP. So I was worried about HRT.

But nothing. No weight gain (maybe lost a bit as aches/pains disappearing since HRT started. And on utrogestan (body identical) rather than sythetic progestin - so no mood changes.

The thing to remember is - you can always come off it if it doesn't work for you. Do give it several months - your body will take a will for all the hormones to be replenished. And you can change doses etc. But you are not signing up for life if you don't want to.

@JinglingHellsBells @PantyMcPantFace
Thanks so much. Your advice is so helpful and has helped me so much.
I phoned the GP this afternoon to get an appointment as my mind is now clearer and HRT seems to be worth a try.
However, I have to phone back at 1030 tomorrow ( and probably every morning thereafter) to get a non urgent appointment....
I've waited 5 years...another 5 weeks won't hurt!
Thanks again, all of you lovely ladies.

I don't understand where the "try to get through menopause without HRT" thing comes from, absent a contraindication such as oestrogen-receptive breast cancer. If you had a thyroid deficiency, you'd take thyroxin. If you have an oestrogen deficiency, why not go on HRT? The benefits in terms of bone health and the other benefits far outweigh the slight increase in risks.

I started HRT as soon as my perimenopause was confirmed by the doctor (at 45) and I'm sure it's saved my marriage. I was an absolute rage-ball before.

I’ve just got the Mirena coil, and as a happy side effect it massively reduced my rages.
