HRT for prevention but don't like side effects

I am on HRT for prevention. I have had no menopause symptoms at all but HRT has given me really sore boobs, pelvic pain and joint aches. I felt so well before HRT but GP says it is vital I stay on HRT as I started menopause at 40 and I need to protect my heart and bones for the future. I am 48 now and I have friends in their early 50's who are nowhere near menopause so I know I need the protection that HRT offers but the side effects are really troublesome especially as I felt so well and normal pre HRT.

You'll get told here that HRT never causes bad side effects or if it does, you just need to try <insert here 455 other forms of HRT>.

Are you saying that very soon after you started HRT you started having all of those bad symptoms, they have stayed just as bad for 8 years ? I can understand feeling very fed up, especially if you can't imagine anything else could have caused them. Have you tried an HRT holiday to make sure all those symptoms go away?

over50andfab · 31/07/2022 14:42

Agree with Lljkk, have you just started or increased your HRT? It can cause side effects initially but generally they settle down. If you’ve been taking it a while and still have symptoms have you tried switching to a different HRT combination?

Star flower oil can help with sore boobs. Joint aches are more related to menopause itself and pelvic pain - possibly hypertonic pelvic floor - can also happen in menopause - something a pelvic floor physio can help with.

Side effects are very common on HRT @folly115 but often they settle down within 3 months or so.

My consultant says that often women need to try 3 or more types before they find the right one and the right dose.

I'm only guessing here, but your side effects sound very much like peri. It might be you need a higher dose of estrogen or even a lower one.

Have you ever discussed this with your dr? Or been referred 8 years ago as you were having an early menopause?

There is also a trend to offer a more natural form of the combined pill to very young women in peri, although (again) my dr believes that HRT is in fact better for results.

What sort are you using and has the type or dose ever been changed in 8 years?

folly115 · 31/07/2022 14:48

When I was on Ellesse duet about 7 years ago I got no side effects at all until I had to change to Novofem which gave me breast tenderness as well. I then tried the Evorel conti patch as had been on a sequi preparation for several years and although no physical side effects my bleeding became so erratic (bear in mind you shouldn't really bleed on a conti preparation) so I came off and the bleeding stopped that day and has never returned which proved I have reached full menopause.

In a nutshell everytime I try HRT it gives me side effects ,which people who have symptoms say is nothing compared to the way they felt before HRT, but as I felt fine before I really notice the side effects.

Sorry should have said I had been off HRT for 18 months without a bleed completely symptom free before restarting 2 weeks ago.

The types you have used are all identical apart from being either a 1mg or 2mg dose of estrogen. They all contain estradiol and the progestin Norethisterone.

Why did you swap from Elleste to Novofem? (Same thing but simply a different name.)

With the combined continuous patch, bleeding is an expected side effect for up to 6 months . This is until the action of the progestin works fully to stop any lining growing and shedding. This is a downside of combined but it's something that has to be tolerated and it stops in the end, for most women !

Is there any reason you can't have Elleste again? If it suited you it seems a shame you aren't using it now and there is a 2mg conti version.

The breast tenderness often goes away after 3 months and if not, a reduced dose of estrogen is what is often suggested. It's understandable that you are having problems restarting after a gap of 18 months. Also, the joint pain etc may have been because you have come off HRT and will ease once you are settled on the right dose. two weeks is too soon to judge.

Another type to try is an estrogen-only patch, or gel, and micronised progesterone daily (to give the conti version.) With a separate estrogen patch, there is more variation in the estrogen dose and also, with gel, you can use whatever amount works without having to keep going back to the dr to try new pills or patches.

folly115 · 31/07/2022 15:39

Thank you. Elleste was perfect but there are supply issues around here and it seems to be no longer available as I did ask about the conti version.

I know I should give it longer but I feel so well without HRT the temptation is just to stop using it but will I wish in 15 yrs time that I had protected my bones and heart.

Many drs now are prescribing gel/ patches and micronised progesterone.

It might be worth thinking/ asking about that.

If you really don't want to continue, your GP ought to arrange a DEXA scan - bone density- and repeat every 2 years. Up to the age of 60, HRT can be started for bone preservation and treatment if your scan showed a lot of bone loss.

You did have 6 years on HRT from what you say- from 40-46, and now 18 months off it.

That still leaves you in the 'early' age range ( 51 is average age for menopause) but the HRT you used will have helped your bones and heart already.

I've had DEXA scans every 2- 3 years (not on the NHS) as I had issues with my bones and they really are invaluable, so you know what's going on.

As you had an early meno, you ought to be offered these on the NHS- ask your GP!

For anyone using Evrol Conti patches. What side effects are you experiencing

hi, why are you specifically asking about side effects which, if we experience any, can be different for all? Might be better to share any you’re experiencing yourself and also if you recently started or changed your HRT.

Bradle · 08/08/2022 18:31

Heavy bleeding every two to 3 weeks on Evrol Conti. Not what I anticipated. Was told slight bleed or none at all due to fact I hadn’t had a bleed for 3 & 1/2 yrs.

over50andfab · 08/08/2022 23:52

Ah I see. Bleeding can happen when HRT is started or changed/increased. If it doesn’t settle it should be investigated under the 2 week pathway,. We’ve seen bleeding happen with quite a few women on Evorel Conti.

If it doesn’t settle and no other cause is found then it might be a good idea to change the HRT using the oestrogen part as a patch, gel or spray and the progestogen either as Utrogestan or the Mirena coil. This gives more control in adjusting how much of each is used.

Bradle · 09/08/2022 06:22

Thank you, will discuss with GP

Bradle · 13/08/2022 16:07

I hadn’t bled for over 3 yrs and having very erratic bleeding with Evrol Conti.

Bradle · 13/08/2022 16:07

I hadn’t bled for over 3 yrs and having very erratic bleeding with Evrol Conti.

Conti can be a pain for bleeding. If it doesn’t settle after several months and scans show nothing else as the cause maybe change to transdermal estrogen (patch, gel, spray) and micronised progesterone (Utrogestan) or the Mirena coil which is effective in controlling any bleeding.

Kimlolly · 14/08/2022 16:15

Hi I’m on evorel conti been on for four months now was previously in them 3 months before but had to stop while bleeding was investigated and last five weeks suffering really bad bloating constipation and windy to the point of thinking of coming off hrt it’s really making me feel down never had a issue with my stomach and exercise and eat healthy but then need to really be on hrt as been diagnosed with osteoporosis back in November anyone else had this happen does it get better

over50andfab · 14/08/2022 23:46

@Kimlolly bloating, bowel and gut problems can be more related to menopause. There is some tips here that might help balance-menopause.com/uploads/2021/09/Bloating-and-bowel-problems-in-the-menopause.pdf.

if the bleeding continues and nothing is found as the cause from the scan it might help to change your HRT
