At 57 and many years on hrt my by gynocologist told me in December to stop hrt until I had my polyp / fibroids removed I was off hrt for 8 weeks worst weeks of my life struggled terrible without it in Feb she gave me the go ahead to go back on it my gp advised me to try oestrogel / ultrogeston as its a safer hrt, so nearly 5 month's on this and have either been bleeding or spotting all the time but I felt good besides the bleeding I take it continuous, after having another heavy bleed while away on my holidays I spoke with gp when got home and as I take my ultrogeston vaginally 100mg she told me to go back to taking it orally and to up my oestrogel to 3 pumps as I was on 2 and a bit we have tried 200mg ultrogeston in the past and just made bleeding heavier, so a week ago I upped to 3 pumps since then I have been a right bitch horrible to husband and then bleeding have started again headaches and wake up with terrible stomachs cramps then I changed to orally yesterday with the ultrogeston and god I can't cope with how I feel today been dizzy and feeling sick all day feel dreadful , don't know what to do I can't cope without hrt got oestroarthrithus and hrt helps with the joint pain but can't go on like this please don't recommend mirena I won't have one gp is pressuring me to have it I really wanted this hrt to work any recommendations what could be going on or what am.I doing wrong I am at the point of just stopping it now but I know my life will just go down hill.

Can you go back to the version you were on earlier this year?

Jan1234 · 19/06/2022 21:31

Yes I was on kliofem 2mg /1mg continuous

I found taking utrogestan orally gave me much heavier bleeding and much worse moods/depression than taking it vaginally. I’m not sure why your GP thought orally would be better! I’d go back to vaginally and hope the spotting settles down.

Jan1234 · 19/06/2022 21:42

I asked her if I could go back on kliofem she said it probably won't work either and she's keeps saying this one now is safer she's actually frightened me to going back on a synthetic hrt now, as for going back vaginally I've been doing it that way for nearly 5 month's and have not made a difference with the bleeding that's why it's my last try to see if orally will work better only just changed to orally but don't think im going tolerate the dizziness and feeling sick

Utrogestan never really worked for me so I had a mirena inserted at just a week short of 57 and it has been much better, stopping bleeding (am on continuous Oestrogel).

Iheartmysmart · 20/06/2022 10:00

I had a similar problem with the Oestrogel and Utrogestan combination. Despite being over 12 months period free I kept bleeding randomly using both continuously. In the end I started taking it sequentially so have a very short but predictable bleed every month which I find easier to manage. My GP also pushed me to try the mirena but having had an awful time with the mini pill I didn’t want to go down that route.

Jan1234 · 20/06/2022 10:10

No i dont want a mirena either i really dont want to go back to having periods but i might have to if i want to stay on hrt how do i use this to go sequel

OP posts:
Abra1d1 · 20/06/2022 10:16

I was adamant I didn't want a Mirena until I was told it was that, have a hysterectomy or come off HRT.

It's been much much better than I expected.

Jan1234 · 20/06/2022 10:25

Ive heard so many bad stories about a mirena and i really am scared to have one i know from friends whos had one once they are in gp will say gjve it 6 months to settle and im scared that il get bad symptoms from it and they wont remove it, ive bled and spotting niw for 5 months cant go through all this again

Ive also been told by gp mirena coil or come off hrt or try sequel i really think its just this one im on was on kliofem be4 never had this problem ive asked her if i can go back on them and she said they wont work i dont think she wants me to go back on them as they are not body identical

If you want to use it sequentially then use the gel daily and for 12 days per month take 2 utrogestan capsules each day. I start mine on the 1st day of each month so I know where I am.

I found the mini pill affected me very badly mentally hence not wanting to try a mirena. Same doubts as you with getting it removed if I wasn’t happy.

@Jan1234

-Estrogen makes the lining grow
-Utrogestan prevents the lining growing.

You have increased the gel to 3 pumps so your womb lining will get thicker than on 2 pumps.

MORE Utrogestan will not make you bleed more. The opposite is what happens.

You could use 200mgs Utrogestan for 12 days a month. That will give you a predictable withdrawal bleed once a month.

Using it every day- as you are now- is not controlling your womb lining. And by the way, it takes at least 6 months for continuous to work properly.

It's ridiculous for your GP to say Kliofem doens't work. Of course it works.

She's just trying to guide you by saying body identical estrogen +progesterone is slightly safer, long term.

Try using Utrogestan 12 days (or 14) per month. Drop back on the gel to 2 pumps which is the normal dose, and see how that goes.

Also, are you taking Utrogestan orally at night before bed? That's when it has to be taken. If you are taking it daytime, that may be why you feel terrible on it.

Jan1234 · 20/06/2022 14:11

Yes i am taking it orally nkw at bedrime and have allready dropped back down to 2 and a bit pumps, im in so much pain today in groin and stomache and headaches when i upped my dosage to 200mg last time cause of bleediing my bleeding just got heavier i just dont think this one is working for me ive changed different hrta in the past and never bled or spotted for as long as this

Jan, you say you were on HRT for a long time.

How long?

Had/ have you own periods stopped for good? And when?

Sometimes this bleeding happens in women who are not post meno and they use continuous too soon.

Jan1234 · 20/06/2022 14:43

Been on hrt for nearly8 years i was on sequel until 2019 and my periods was so painful that i went on to the conti my periods never stopped by themselves i was 49 when i went on a sequel because i was bleeding every 10 days im nearly 57 now

I'm 54 and on continuous HRT. Utrogestan and 2 pumps of oestrogel. I started 6 months ago and have got on really well with it but recently started a very light bleed now and then which lasts a few days then goes away. No other problems. Spoke to a doctor yesterday who wants me to stop the HRT, have an ultrasound and blood tests which has put the frighteners in me as you automatically think of cancer! I feel this is a bit over the top? Sometimes I take my HRT at different times, always in the evening but it could be 8pm, 9pm or 10pm etc. When I'm late with it I notice a discharge the next few days. Is this the reason? I'm confused and not sure what to do. Can any one help please?
