Mirena plus additional progesterone

Hi...sorry, long one...
52 and been on everol patches for 3ish years and mirena coil for 2.5 years. Prior to coil, periods were horrific. Coil sorted all that...but...every so often, I'll have spotting...def not a period but say 10 days of nonsense and not even having to wear a panty liner it's that scant. It's just annoying more than anything.

When I discussed this with my meno nurse, I was on a 75 patch and at the time and she suggested taking an additional progesterone tablet. I did some research which indicated the bleeds could be because i was estrogen heavy so I chose to reduce the estrogen back to 50 and not have the additional progesterone.

Seemed to work, no bleeds and carried on like this for a year but then last month, all of a sudden I started to spot again but this time round I had awful pmt symptoms; moods were very erratic and felt like I could kill someone if they so much as looked at me. Have packed on weight too despite no change to diet/exercise.

I'm waiting for a call back from specialist nurse but just wondered if anyone else had been prescribed additional progesterone as well as having coil? Worried that I'm getting symptoms of progesterone intolerance so adding more to the mix could be bloody awful!! 😕

P.s. have had 2 x scans and there is nothing sinister going on in the womb, just a small fibroid..

Wish i could offer advice but struggling here too.

Had coil fitted 12 days ago, so far awful cramps, headache, nausea..
I only got it to try and tackle bleeding on hrt ( utrogestan didn't work ) but if this is what I've got to put up with, the coil is coming out!

Are you sure these aren't symptoms of low oestrogen rather than progesterone intolerance? Are you absorbing the patch okay, I only ask as I'm also on the 75 patch and have been for a while, but feel I'm not absorbing it now. Maybe arrange a blood test to check!

The coil would have settled by now for you, but if you've become progesterone intolerant it might be better to get the coil out and try something a bit gentler until you can formulate a plan with your specialist.

Too me, it's either low oestrogen or progesterone related.

Sorry not to have any words of wisdom, but jingling might reply and she's VERY wise and knowledgeable on these matters!

Sorry you're having issues too. It is utterly shit right now. Last 2 years, symptoms haven't been too bad but right now I just feel so crap. It's so rubbish!

You could be right, that I need to up the oestrogen but I worry, that if I do, it'll set of more spotting/bleeds. But if it means I feel better and other symptoms are reduced/eliminated then I'll be happy with that!

I just thought it was a bit much to have more progesterone on top of the mirena too...tried googling but can't find any kind of reference to this being a normal thing to prescribe.

I'm hoping for a call back from meno nurse 🙏 to discuss further but it's all just a guessing game really isn't it. Just grateful there are other options to consider!!

Hope you get some relief soon...it took a month or so for the coil to settle for me so hang in there!

Yes it's not usual practice to add more progesterone to a mirena I don't think, as it's slow release and therefore should be effective at keeping the lining thin and healthy.

Definitely get a blood test to check oestrogen absorption, as I'm not sure why you would get PMT symptoms now, unless low oestrogen or some kind of fluctuation.

I agree, it's shit! My MS is a fan of Mirena and also says stick with it also, but it's hard!

Hope your nurse specialist helps you move forward with it all ( and doesn't take too long getting in touch !) x
