Bleeding 4 years after menopause

I think I have to call the GP with this question but wondering if any other women have had similar.

I had my last period at 53 years and 8 months. Nothing at all since then. So am now definitely post menopausal as I am 58 next month.

Three months ago I decided to see the doctor about annoying menopausal symptoms of hot flushes especially at night which had wrecked my sleep for years. As well as brain fog and vaginal dryness.. (all the usual stuff). The GP suggested I try a HRT patch and prescribed Evorel Conti patches two a week. It's been amazing. Have been sleeping through and feel like a different person. Definitely have some of my v v voom back ! However when I went to the loo today I had some blood in my pants. I wiped and had the same kind of small clots I used to get with a period. It's still there but now brown and not anything more than enough to stain a panty liner.

Is this normal on HRT or is it something that needs checking out. I am a bit worried as a close friend started bleeding after menopause and it turned out to be uterine cancer. (She was treated successfully thank goodness but it is on my mind)

Anyone had something similar? Would hate to stop the HRT as it has been life changing.

As far as I’m aware if you’ve been through the menopause and start bleeding again it’s a straight to the gp moment. May be nothing but they will definitely want to see you op.

I agree, see your GP! Have you recently had a covid vaccine? There are many stories of post-menopausal women bleeding after the vaccine, but either way, you need to get it checked.

I thought as much... didn't want to be unnecessarily paranoid if it was a common side effect of starting HRT .

Not to alarm you, but I have definitely been advise that any bleeding whatsoever after menopause is a possible sign of cervical cancer. Please go get checked asap (hopefully it's nothing!)

I'm 68 and have had this for a while ( have to wear panty liners as it's a constant trickle) I was fast tracked to a brilliant clinics who did a hysteroscopy and took endometrial biopsies. It all came back clear but I have a huge calcified fibroid which is causing the problem. I've been offered a hysterectomy. Haven't made my mind up about that yet!

Agreed. I attended a talk on menopause just last night and the gynaecologist said exactly this. She also said it might be nothing Smile

This happened to me 3 weeks ago.
It's not uncommon but you do need to tell your GP and they will fast track you for further investigations on the 2 week suspected cancer pathway which is a bit alarming. You will be offered an ultrasound, probably a transvaginal ultrasound and probably also a hysteroscopy.
I am currently waiting for the hysteroscopy which would have happened in clinic yesterday but I requested anaesthesia so it will happen in a couple of weeks time.
It's most likely not cancer of course but it needs to be checked out.

Also yes it could definitely be due to the HRT. I have recently gone up a dose which my nurse consultant advised is likely to be the cause. They still want to do a hysteroscopy though.

Definitely get checked. But I've read a few reports of post-menopausal women starting to bleed again after they've had Covid vaccines. Look it up - it's a thing!

Yes my bleeding started 3 weeks after the booster. It's impossible to tell if this is coincidence though so it still needs checking. I reported it on the vaccine yellow card online reporting scheme.

I’ve been bleeding on and off for years (I’m 68, also on HRT including a Mirena for the progesterone) and have been throughly checked over - it’s nothing bad. I’d have liked a hysterectomy but they said no need (this is a very highly regarded NHS menopause clinic).
But see the GP anyway. It needs investigating.

Op, glad to hear you're speaking to GP.
I had similar in Aug 2020. I was referred to hospital (2 week rule) and had a transvaginal scan followed by the removal of a cervical polyp. All good.
Starting Oct 2021, I had constant low grade pain in the pelvic region and then some spotting in Dec. Once again referred to hospital where I had another tv scan which found womb lining had thickened to 8mm so this time I had a hysteroscopy. I'm still waiting for the results but consultant found and removed a polyp and also thinks I have a fibroid which means I'll have to go back again for removal.
I hope yours is straightforward.

GP explained that the initial route is to come to the GP for an internal exam on Monday. (Oh joy) . . Just to check that there is no internal tear or polyp inside my vagina/problem with cervix If all looks fine then I will be referred for urgent trans vaginal ultrasound and go from there.

This happened to me last month. Ultrasound confirmed a thickened endometrium by quite a lot and a cyst on one ovary.

Had biopsy done and no malignancy. It was concluded it was just 'hormones' and nothing to worry about.

Hope you're well OP and this is just due to the hrt.
I know post-meno bleeding is meant to be looked into but if it helps I just started hrt a month-ish ago and started bleeding two weeks ago, emailed the meno clinic and they said it's normal when starting hrt and they would like me to keep an eye on it and we can discuss at my next appointment in April. Slightly different in that I was barely out of peri (a scant year since my last period) and I had an ultrasound and hysteroscopy last year due to heavy bleeding.

Hi OP I'm the same age as you and finished menopause with no symptoms about 4 years ago too. Big difference is I've never needed HRT.

But, I did have a small bleed after I had my booster in December. I was put on the 2-week pathway and had a scan a which showed a very thickened endometrium (18mm).

I had a hysteroscopy with biopsy 10 days ago. I was told there and then that I have endometrial cancer. I have an MRI booked for this weekend to check it's not spread then a first appointment with a gynae oncologist next week. I'll get my full results & formal diagnosis then, and a treatment plan (hysterectomy first, possibly radiotherapy depending on the stage and extent of the cancer).

Please carry on with the process. I hope you're OK and it turns out to be nothing, or at least something easily fixable, but don't take the chance.
