Utrogestan Vaginal application

Parques · 18/11/2021 09:30

Looking for advice ladies! Is it correct that Utrogestan 100 (small white tablets) can be inserted vaginally? I am taking them orally at present. Do you need a special 'vaginal Utrogestan' or is it the same tablets regardless of oral/vaginal use.
Any experience/knowledge much appreciated. Thank you.

@Parques Vaginal use is not licensed in the UK, so there is no 'special type' anyway to do it. If an NHS dr suggests using it vaginally, it's 'off licence'. If a private consultant prescribes it outside of the NHS they can do whatever they like.

Having said that, women do use the 100mgs vaginally. It's simply a case of the licence in the UK not yet up to date with how it's used elsewhere in Europe (vaginally is the norm there.)

If you are using it on a cycle, you need to use 2 capsules each of the 12 days. If it's a continuous regime (post menopause) then it's 1 capsule daily.

A 200mgs vaginal capsule does exist, but on the NHS it would only be given as fertility treatment. It comes with a plastic applicator.

I have had the 200mgs version for vaginal use (it was a private prescription) and TBH the applicator is not worth the effort.

I use them vaginally and thought they might be difficult to insert, but they're not.

MammaLovesLeopard · 20/11/2021 05:59

I've gone with using 100mg vaginally as this was suggested by a Dr. at the Newson Clinic

  • who said that since you are putting them nearer to your womb when using vaginally, 100mg would do. I talked to my GP about this, and she thought that made sense.

Since taking 2 orally made me feel so horrendous, I figured I'd start with 100mg and see how it goes.

Popped the first one in last night. Really hoping this works 🤞

Parques · 20/11/2021 09:06

Thanks mammal. I’ve inserted for 2 nights now. No ill effects so far! 🤞🏽🤞🏽

I seem ok this morning @Parques
in that I am not hyperventilating with anxiety, as happened when I took orally.

I do feel a bit premenstrual & I slept badly, but that may also be because I had a few glasses of wine last night at a birthday party.

Will stick to water tonight & see how I go! 🤞🤞🤞

This is great! 👏
MammaLovesLeopard · 21/11/2021 15:16

Today I feel that my digestion is not great. My stomach is uncomfortably swollen.

DinoDora · 22/11/2021 20:23

Thanks for this info, GP actually prescribed this for me today, despite off licence.

I was in shock!

DinoDora · 22/11/2021 20:25

She said two tablets in vagina though so I'm not sure what to do.

It was up to me whether I do orally or vaginally. I'm not sure I can faff about with taking away from food.

MammaLovesLeopard · 23/11/2021 03:06

I'm a few days in.
It's better than orally.
But I'm not great.
Actually awake right now feeling a bit itchy at 3am 😢
I am really wondering about patches tbh.

DinoDora · 23/11/2021 05:11

Sorry you're feeling rough.

Doesn't that sound like not enough oestrogen?

I'm feeling the same without hrt Grin!

MammaLovesLeopard · 23/11/2021 07:29

Thanks @DinoDora I was wondering about upping the oestrogen.

The itch was where in had inserted the tablet. It doesn't feel great n there tbh.

I'm going to see if I can get to speak to my doc. But its early days I'm still on my first month.

Parques · 25/11/2021 23:29

Seven days of inserting vaginally and my breasts are so sore. Going to give it a few more days then might revert back to oral injection. Perhaps it’s ‘too much’ if used vaginally?

MammaLovesLeopard · 26/11/2021 07:53

I have also done 7 days but cut dose down to 100mg as suggested by the Newson clinic. Ran the idea by my GP too who said it made sense. I have found it much easier to tolerate. Maybe try one capsule instead @Parques ?

MammaLovesLeopard · 26/11/2021 07:55

@Parques maybe discuss with your GP?

Thanks Mammal. I’m only on one tablet. I’ll persevere for another week and then will perhaps switch back

Hi there I managed 3 days this week starting hrt. I did my third utrogestan vaginally and I've got vaginal atrophy anyway which this has now exacerbated.

I was a sedated wreck on those 2 days of utrogestan orally, really shocked me.
I chose utrogestan because it's meant to be more identical. But I cannot tolerate it.

I'm going to have to speak to yet another GP about progesterone options.

Parques · 27/11/2021 08:39

It’s horrendous isn’t it? A complete minefield Mammal

MammaLovesLeopard · 27/11/2021 10:24

I seem to be tolerating the Uterogestan but have not noticed an increase in libido whatsoever.

I feel calmer. I've lost 6lbs. So those are good things. I'm thinking i might increase the oestrogen & see about adding some testosterone, although this will have to be privately prescribed I understand

crispinglovershighkick · 28/11/2021 14:31

Don't want to step on your thread OP but like you I'm finding the whole thing a minefield. It took so long for my meds to be in stock that I've forgotten almost all of the phone appointment I had with the newly menopause-trained GP almost two months ago.

I've read here that vaginal Utrogestan is off license for hrt but I just got mine in the post with the enclosed leaflet entitled Utrogestan Vaginal detailing how to use them vaginally. Is it that there are different types?

I'm 54 and this is deffo not fertility treatment Grin

JinglingHellsBells · 28/11/2021 14:44

@crispinglovershighkick There are two options with utrogestan

1 the 100mgs which is most frequently used as women take either 100mgs daily or 200mgs (2 capsules) for a few days each month.

2 the 200mgs version which is exactly the same as the above, but twice as big.

The 200mgs version is also used for fertility.
It's not actually licensed for use as HRT on the NHS (in that size) which is why most women now are getting the 100mgs and using 1 or 2 capsules, depending on the dose for them.

I have a private prescription for my HRT and pay for each capsule. The 200mgs are very expensive (more than 2 x the 100mgs) so I always ask for the 100mgs.

If someone has given you the 200mgs that's fine, but of course if you are using it daily, then it wouldn't work as the daily dose is 100mg.

crispinglovershighkick · 28/11/2021 15:02

Thanks Jingling for the very quick reply. (I've started my own thread so not to step on yours too much OP.)

So when you say 'daily dose' I'm meant to take Utrogestan 200mg (along with Estradot 50mcg) for two weeks of 'cycle' (which I no longer have naturally, having had my last period in January). It was prescribed on the NHS, I'm not sure how it works if not licensed, but it's not a private scrip.

Thanks again.

If you are less than 12 months from your last period, you will use this sequential regime of 12 days per month ( a lot of drs are saying 2 weeks but the leaflet in the pack says 12 days and that is enough.)

For women post menopause ( 12 months or more) they tend to prefer the daily dose of 100mgs that means no withdrawal bleed, monthly.

Utrogestan is licensed as HRT but the 200mgs was phased out and women were being given the 100mgs.

The vaginal use is 'off licence' and although many GPs are now happy to take this on (it's their choice as it's off-licence) they would normally put on the prescription the 100mgs size x 12 days ( so 24 capsules per month.)

If your GP has put 200mgs per cycle, and you've got that size, all well and good. It's not a problem.

Cost-wise they are ££ as they come with a plastic applicator (well, mine did) so I no longer ask for them!

Parques · 01/12/2021 22:47

Fed up now! Sore breasts and now bleeding too! I haven't bled for I don't know how long! Also experiencing night sweats every night! Any advice about increasing oestrogen?
