I visited a gynaecologist last week to discuss bladder issues - constant weeing / urgency of peeing etc and the first thing she said was the hrt dose I was on is too low. I’ve been on it 18 months I’m nearly 51 - perimenopause. I’ve been on 1mg sandrema and progesterone day 15-26. She suggested doubling gel to 2mg. I’ve done this for the last 3 nights. Last night I woke up with palpitations and what I think was a mild panic attack and I feel rotten. Is this just a coincidence? I thought it sounded a good idea as recently I’ve been waking up hot which was a new symptom. Hrt has been good to me helping my mood, confidence, anxiety, irritability and fatigue (a bit)
Any advice welcome- thanks

When I had a virus that was possibly covid back in March, it started with palpitations and panic attack feelings and moved on to diarrhea, loss of appetite, dizziness, sore throat, insomnia, and feeling like I'd inhaled talcum powder then a cough. Then weeks of indigestion. Couldn't get tested back then but the panicky feelings were so strange as it really felt like severe anxiety but it just came on so fast and then suddenly went away.

So if you haven't had covid just bear that in mind as a possibility alongside the change in HRT. I am sorry I don't know how changing that might affect you but the panic and palpitations sounded so like my possible covid experience.

Incidentally I am 50 and also in Peri, but hadn't started HRT (was about to) when I fell ill.

It was a strange illness but never had me flat out, I WFH throughout.

@Calmandsteady

Was this gynae and meno expert? The thing is, the usual treatment for bladder symptoms (which I have had for years) is vaginal estrogen cream as it goes right to the spot.

It can take up to 6 months to really work but not always that long.
If you have other peri symptoms then full HRT is good, and some women need full HRT and vaginal treatment to help bladders. BUT the guidance is always use vaginal estrogen first/ alone to see if it helps with urinary issues.

I think it's pretty obvious that jumping from 1 to 2 mgs is too big a jump and that's why you feel a bit weird.

I'd go back to whoever you see and ask for Ovestin or Vagifem for the bladder issues and maybe use 1 and a bit sachets of gel and increase graduallyif you need to.

Thanks she has prescribed vagi fem too - every day for 2 weeks then 3 times a week not collected it yet - the sachets I use are 1mg in size so I guess maybe I could split them to use 1.5 ? I started the night sweats about 2 weeks before She’s a private specialist that I went with through health insurance she does specialise in meno too and was great - a real advocate for hrt. So I’m not seeing again for 3 months

I think maybe not use the whole of two sachets.

Incidentally, private health insurance doesn't cover menopause once your symptoms have been diagnosed as menopause and being given HRT. Presumably you went to her for other things and she said they were meno related?

I've had private cover for years and it did cover things that were linked to menopause, for a diagnosis, but once I was into a routine of seeing my consultant for check ups on HRT I had to pay, and still do.

Calmandsteady · 17/06/2020 12:39

That’s right I was seeing her about bladder issues - this topic just came up and she prescribed vagi fem As likelihood was that it’s because of low estrogen levels- all meno stuff goes through my GP.
Did vagifem help your bladder symptoms jingle ?

I use Ovestin which is a cream, same sort of thing, and yes, it does. I felt most improvement after about 6 months and shortly after that went onto full hrt for other symptoms.

Calmandsteady · 17/06/2020 17:49

Thanks Einstein and jingles really helpful

I’ve just increased sandrena from 1 sachet to 2, and have been feeling really anxious and jittery, heart palpitations, waking up in middle of night in a panic etc. I think it’s to do with increased oestrogen

Calmandsteady · 18/06/2020 17:24

Thanks - I cut it back to 1 last night and didn't have the symptoms at all - so going to stick with 1 sachet and the vagi fem local oestrogen and see how I go !
