Ok, so I'm mid 50s and last period was 4.5 yrs ago. (I had pretty heavy periods when they were regular). Have pretty much had a symptomless Meno and am fit & well.

3 yrs ago I had unexpected spotting; literally saw a bit of fresh blood when I wiped and that was it, just that once.
GP referred me to gynae & had u/s & some sort of womb biopsy (can't remember what they called it).
All was ok.

Two weeks ago, I woke in the night, horny as hell and had sex with DH. We rarely have sex, it's fine, doesn't bother either of us. It was kind of like I'd ovulated though, I was that horny!

A few days later, saw a dab of blood when I wiped. Just the once, no more blood that day after wiping. Thought the sex had possibly caused it.
Today again (no sex since last time 2 wks ago), red fresh blood, just a spot on the liner. No more through the day after seeing it the first time.
I'd had slight period type pain this morning prior to seeing the blood.

Have spoken to GP who has already referred me again for u/s etc.
He mentioned that last time the gynae specialist had said something about using topical oestrogen if I had more bleeding but suggested he referred again as it's been a few years & I'm in agreement with that.

My question is: what does topical oestrogen do?

I think they thought last time that the bleeding may have been caused by sex but I don't remember that we'd had sex when it happened 3 yrs ago - we really do have sex that infrequently I would say we probably hadn't.

It's so weird, with the ovulation type feelings etc.

Do you have pain or discomfort when you have sex?

Vaginal estrogen reverses changes that happen in the vagina after menopause.

It restores the natural pH of the vagina and plumps up the cells that line the walls.

If you have sex, thin walls that are also dry, can bleed and tear. You might not feel it but you might have pain during sex.

You need to have your scan first and be sure nothing else is going on.

Topical vaginal estrogen is not just for women who have sex.

It's used by very old women too, to stop bladder issues and prevent infections.

It's normal to use it for years and years to keep everything comfortable.

Update:

Had spotting on Monday last week & spoke to GP. Referred that day, had u/s on Thurs, which showed nothing of concern, lining very thin, ovaries normal & saw consultant on Fri who said they were happy all ok but did offer biopsy which I accepted & had done there & then. Just got to wait for results now.
Consultant thought it could have been caused by a fibroid that is degenerating.

Amazing service, feeling very lucky I live in a country where this is available - and whilst all this Covid-19 shit is going on too.
