Menopause

Hi - I’m 49 and have been peri menopausal for some years. I suffer from migraines and two years ago my GP talked me out of having HRT thinking they would worsen. Recently my night sweats and flushes have become unbearable and lack of sleep is a major migraine trigger for me !!
Yesterday I went to see my GP well informed with literature/evidence.The GP was sympathetic and listened however was unaware of the micronised progesterone. I felt embarrassed and apologised but showed her my information and gave her my reasons for wanting a oestrogen only patch and micronised progesterone. She was cautious but obliging. I’m seeing her again in one month and she says she will be looking at your guidelines before my next visit
However I’m seeking advice on the utrogestran. My last period was 6 months ago. The GP has adviced 100mg at night day 1 -25 - does this sound ok. I know some women use 200mg for 12-15 days.
I’m also starting Evorel 25 patch.
Thanks

Well done for coming away with the HRT!

Your GP is wrong.

There are a lot of issues to think about here.

Continuous regimes (that is both hormones daily) are for women with no periods for 12 months or who are 54+.

So you should be using the sequential regime which means it follows a natural 4- week cycle.

That is micronised progesterone for 12 days a 28/30 day cycle.

This means you could start it on the 1st of each month for 12 days, or the 15th of each month- any day really, as long as you stick to the pattern, approximately.

This will result in false period about 3 days after the last Utrogestan capsule.

The other thing is, 25mg patch is very low for someone like you in peri and under 50. It would have been better to start on something like 37mgs or even 50mgs.

I use oestrogel and the amount is equivalent to a patch of around 50mgs- I used that at age 53 when still having occasional periods.
I suspect you will not get total relief from 25.

Also- !!!- using daily Utrogestan will 'wipe out' (potentially) the benefits of that small amount of estrogen in that size patch.

Finally, your GP should know that the first review of any HRT is after 3 months, not a month. It takes longer than a month to see if it's the right dose.

@jinglehelksbells. Thanks for your reply. Tbh she was very willing but didn’t have a clue and think she felt very uncomfortable that’s why she asked me to com back in a month

I thought the patch would be in the low side but I’ve started it today and will ask for an increase when I go back. As I’ve missed the 1st this month I will start the Utrogestan on the 15th. I was hoping I could take it daily and improve my sleep with its drowsy s/ effect 🙄

Sorry jinglinghellsbells is this 15th day of a calendar month or a cycle. If a cycle do you count the first day of the bleed as day 1. 🙄
