DS1500 form

My lovely Dad has recently been diagnosed with stage IV liver and lung cancer. At a recent oncology appointment when he was put on palitive care it was suggested that he applied for attendance allowance.
A form from the drs has arrived with the drs part already filled in, we had to put Dads bank details. There are only 3 pages but it doesn't say who to send it to. In fact on the page the dr has filled in with his diagnosis it states at the top This is not a claim form and also Patients copy.
I left messages with the cancer nurses and sw to call me back yesterday but I don't expect them to call me back over the weekend and I'd really like to post this off tomorrow.
If you have posted off this form or are in the prosess of doing so please can you let me know where to send it as google is giving me different addresses.

You need an attendance allowance form. Ring the attendance allowance helpline and they will send one out and your dads claim will start from the date you call. The DS1500 is to support your dads claim to benefit because it will have your dads diagnosis on it. It will mean that his claim should be looked at quicker and special rules should apply.

24User13052017, The form does say DS1500 patients copy, the dr has filled it in with Dads diagnosis. Why would he not send the whole form to us, I mean what else is there to say, it doesn't make sense. Dad spoke to his cancer nurse and she told him to post it to free post attendance allowance, but I'm not sure he's got that right.
BWC I will see if i can contact them over the weekend.

The doctor wouldn’t have copies of the attendance allowance claim form, they come from the DWP. Make sure you tell them that you are applying under special rules when you ring, then they will fast track the claim. The claim form will have lots of questions about the care and help your dad needs as that is what decides the level of award

You can print off the Attendance Allowance claim form on the government website here:

The DS1500 is the medical evidence form you would send in with the completed Attendance Allowance form.

You can complete the form on behalf of your Dad. You can get help filling in the form from CAB or Age Concern or MacMillan / palliative care team.

Keep in mind when completing the form that it is an assessment for 'personal care', not general help in the house, though once it is granted the money can be spent however your Dad wants.

With a DS1500 form, there is no need to complete the individual questions about what the claimant can/ cannot do. The specialist evidence is sufficient to passport the claimant to the a higher rate AA award.
