Changing my child's name by deed poll - how?

Hi,

I would like to change my daughter's surname by deed poll (she has my surname but we are changing this to my DHs surname).

Having looked it up online it seems that we can do this without paying a hefty amount to solicitors etc, and can simply write a letter ourselves and get this signed by a witness.

Does anyone know the wording for this?

Any help appreciated..

I did it for DSD over the internet, from memory I think I used these people.

I will give these guys a try. There are so many sites advertising this, but they all look like dodgy scam sites!

As far as I know, you need the permission of everyone with Parental Responsibility to change a child's name, if that's relevant.

I changed my name from my father's name to my mother's maiden name at the age of 11 after their divorce. It may be slightly different in Scotland, but I just had to use it and keep using it. I didn't fill in any forms, but when I was applying for a bank account a few years later, I had to get a letter from my school confirming I had used my mother's maiden name for X many years. Everything, until I got married, was then in my mother's maiden name, including passport, NI card, degree etc.
