So I’m in the process of doing this but I’ve got to a stand still with it
This part on the form is optional but having seen 2 people go into care it’s really made me think of what I would like etc if I didn’t have capacity to speak for myself

things like
I would like to stay at home until things become to hard for myself/partner/son
like to be within 5 miles if my son/partner (if he want/can do a visit)
id like to hair regular haircuts/ twice weekly
blow drys

what would you ADD l

I think it is good to get your wants onto the POA as by the time it is needed you will not be able to do it. Are you also going to put in what sort of intervention you want?

oh yes - I do not wish to be kept alive if/when I am bed ridden and in pain or CPR if I have bad dementia etc

just thinking what else I’d put on form that I’d like doing/want if in a care home

ladies what would be your wants ??

The trouble is you saying you don't want to be kept alive if you have bad dementia would only apply with a DNR (do not resuscitate order being placed on your GP record) .

And you might be kept bed ridden if you break your hip or whatever. And some homes won't let you out of bed if you are going to walk around and are a fall risk.

Do read the guidance carefully. I usually advise clients to avoid instructions wherever possible and list wants under the "preferences." Fairly common preferences included in Health and welfare powers eg:

  1. If possible I prefer to be cared for at home.
If my attorneys decide that it is necessary for my care needs to be met in a care home, I would prefer to live in a care home which (state important features eg is within 5 miles of where X lives, is near the sea, has a well stocked drinks trolley etc)
  1. I would like to continue to worship in accordance with the rites of the (faith) at .... or to receive the rites at home
  1. If my doctors advise that my condition cannot be cured I do not wish to receive treatment to prolong my life and would like my attorneys to arrange palliative care only to keep me free of pain

Financial LPAs often include things like preferring the attorneys to continue to support specific charities by maintaining charitable donations as long as financially feasible.

But you can put in whatever you want provided you don't inadvertently contravene the overarching appointment eg instruct attorneys to act jointly despite appointing them on a joint and several basis
