Raising a complaint with NHS

A couple of questions:

  1. Can anyone recommend useful templates for a complaints letter to the NHS? I've found a number of examples online, and am unsure what kind of things are okay to include in terms of what you expect the NHS to do (for someone left with permanent disability due to lack of accurate diagnosis). E.g. accepting responsibility for lack of diagnosis, staff training, ensuring common blood tests are taken.

  2. Would a complaint letter need to state the complainer wishes to retain the right to seek compensation if appropriate?

I'm trying to help a friend so don't feel able to share any more specific information.

There should be a complaints process outlined on the Trust's website for whichever hospital, often under feedback or PALS. You don't need a template just clearly state who you/patient is, what the date and circumstances were, what your query/complaint is and how you want a response. Do you have the relevant patient notes, if not these are available by making a DSAR.

And on point two, no there is no need. The process of making a complaint (and it being investigated) is separate to the decision to seek compensation/consider legal proceedings. The charity AvMA might have some useful information on their website for you and your friend.

I have already got the complaints procedure, and my friend has some of their records via a SAR.

It looks like there's lots to read on the AVMA website - thanks again, we'd never heard of that organisation.
