Deleting Innacurate Medical Records

Trying to help a friend so I said I'd post on here : Her adult son had a psychiatric evaluation/drug review with a new doctor. Several major innacuracies were contained in the report (some of which would have terrible implications for her son if a child-residency battle ensued).

The report also stated that my friend had extreme mental health issues, which is also not true - and some names and dates were also wrong. This report was sent to her son's GP who was already unhappy that the family were seeking advice elsewhere.

Where/how should she begin to remedy this situation if that was at all possible?

She should get her son to write to his GP and the doctor who wrote the report saying what is wrong and providing any supporting evidence he has. It is unlikely he will be able to get this information removed but it may be possible to get a note added explaining what is incorrect and why it is wrong.

Yes, I've never seen removal of entries from records, only notes of correction.

I tried for years to get incorrect information placed by adults when I was a child in their care, removed. It was put there to cover things up.

All I've ever been able to do is add a letter denying the conditions, any need for medication for them, and a supposed hospitalization.

I was told that it's important not to try and explain why you think someones put incorrect stuff onto you, just that it is incorrect and any evidence you have.

It can be unnerving realizing you just have to live with factually incorrect stuff when it feels that your own medical records are important, but my experience ever since has been most GP's run a country mile when they realize they're now involved in a situation where another medical professional may have been party to anything controversial or incorrect, and are guarded with contested information.

They won't delete anything, who carried out the sons evaluation, was it recorded on tape, if the doctor involved has made errors and the report is inaccurate then you can speak to your health authority or the GMC if they feel the doctor was incompetent. MiND might have advice on this, do you know who the evualation, can they seek another one. They can write to the psychiatrist and copy in the g.p stating the inaccuracies. If it was done in a hospital maybe the PALS department can help.

I agree with Dogs, it can be approached from different angles with letters, corrections, and complaints.
It's satisfying to know you've exhausted all angles, or at least tried.
But the end result is unfortunately very unsatisfactory.
