Lasting power of attorney - any knowledge?

About ten years ago, I agreed to be the 'first reserve' attorney for my step grandmother, behind my mum (her stepdaughter) and my dad. Thought not much more of it in the intervening years as SGM has been very much in control and pretty healthy, although not much in touch with us.

In the meantime, my mum has since developed Alzheimer's and therefore obviously can't do it, so I will be working with my dad to enact the POA for my SGM if need be. She is currently still compos mentis but is in end-of-life nursing care and likely to need our help soon. She is very tired and sleeps a lot so doesn't talk much on the phone (she lives at the other end of the country). Dad's caring for my mum so it is likely that mostly it will fall to me. I'm happy to do it, but feel a bit under-prepared.

The first thing I need to sort out is that my old address is on the paper work, and SGM has moved her solicitors twice - it feels likely that communication will be difficult and potentially too slow to be helpful to her as we all try to cover the gaps.

Can anyone advise me on how to track down her current solicitors? She was too sleepy to tell my dad when he spoke to her recently. I would also be grateful if anyone could tell me how to update my address - there doesn't appear to be an online form at the Office of Public Guardianship, although we might not have searched hard enough.

I will do online search etc but to be honest Mumsnet is nearly always better than google. I would really appreciate a steer for anything to do with POA and end of life jobs to do. I just want to get myself ready to be helpful.
