Change daughters surname

Hello,

My 16 year old, wants to change her surname, to mine (she’s had no contact with her father for several years).

I have finally received consent from her father, after tracking him down. He’s agreed, but as said all he will do is send me a affidavit that he’s printed off of the yougov website.

But doesn’t the affidavit need signing in front of a solicitor?

She wants to use her new surname on her passport and bank account.

Can anyone advise us on what steps we should take, please?

In England I'm sure at 16 she can change her name legally via deed poll at a solicitors by herself. No need for parental consent.

For the form of the deed: www.gov.uk/change-name-deed-poll/make-an-adult-deed-poll

For the fact that a 16 year old can change their own name www.gov.uk/change-name-deed-poll/change-a-childs-name

For the fact that you don't need to enrol a deed poll www.gov.uk/change-name-deed-poll

No need to go to either a solicitor or one of those deed poll companies masquerading as official government departments.
