Deed poll help

I need a deed poll for my DC. I'm moving one of their middle names to be a double barrelled surname.

E.g.

Sarah (forename) Smith(middle name) jones (surname

To

Sarah (forename) smith jones (surname)

I got a deed poll but it just lists her name without stating which names are fore, middle or surnames so her name looks exactly the same.

Can anyone recommend a deed poll where her name change will be obviously either by listing spurned first for different boxes for forenames and surnames ect.

She already uses the new name without a hyphen so I can't add this in and I don't want to. Her father my husband is in agreement of the name change.

You can just type it out yourself to make it read how you want.

It's for my dcs first passport and the passport office are being really fussy about what they'll accept. It's stating to annoy me to be honest.

It's not an exact science. There are 3 parts to a CON deed.

1.You abandon use of the old name and adopt the new.

  1. State for what purposes the new name is adopted and in what documents.
  2. Authorise and require everyone to use the new name.

As long as you've got all three in it should be a valid CON deed.

My daughter has been using her new name since she was about 6 months old.

The passport office require a deed poll so I just need a recommendation of a company where the forename and surname are clearly shown.

You can only change a child name if you have agreement of both parents with PR. Is this an issue here?
