DVLA medical form

Not sure if this is in the right place, but here goes...

I'm applying for my provisional driving licence, but have had to declare on the form that I have epilepsy. They've sent me a form back, with a lot of detailed questions, asking for specific dates of seizures and medication etc.

The problem is, my epilepsy developed at the age of 9, and my last seizure was when I was 20, so I have no idea for most of the answers!

Can I send the form back partially filled in? Starting to panic as time is running out! I have signed the declaration for them to access my medical records

I think you will need to send the form in fully completed. It is important.

If your last seizure was when you were 20, how old are you now? I ask because you will not get a licence unless your last seizure was more than 2 years before applying.

I know someone whose licence was withdrawn for 2 years because they had had one after they'd passed their test.

Are your parents around to ask about details when you were younger? You need to be as open and co-operative as possible as they will err on the side of caution - I.e. if they think you are trying to cover up recent episodes you will probably get refused.

My parents are around, but I asked my mum and she said she could guess the year for most things, but no way would she know the day or month!

I'm 27 now, so 7 years seizure free. The law states that you have to be free for a year before you can apply for a licence, so I'm easily clear!

That's the thing, I don't want them to think I'm being deliberately unco-operative, but I genuinely don't have a clue! xxx

Complete it as far as you can. Be clear where you are not sure. They will get medical records

I fill mine in as much as I can.
I do complete it. You must.
If you are not sure, say so. Give them a rough date, but write down that you are not sure exactly when.
If it was 7 years ago, no one will really care if it was July or August.
write 2006. ( can not remember month), - write that in brackets.
Then they can never accuse you of lying/falsifying.

I will be fine. Really.

They will also send a letter to your GP so he/she may be able to help fill in the blanks if he knows. As long as you are over (2 years post last seizure....one year is for night time seizures) you'll be fine.
Are you still on medication? If so you have to wait for a certain time after stopping to drive again.

The form states: "If you are unsure of the answers, we advise
you to discuss the form with your Doctor" so I would do that.

oh Blush where did it state that? I must have missed it. I filled it in as best I could, then put in a note explaining the gaps.

Yes, still on medication. After being told I had childhood epilepsy (which has almost always been well controlled), and that I could wean myself off of my tablets, never again!

Whether it's one, or two (I have always been told one) years, I am definitely safe xxx
