Separated and taking children out of the country

I have a friend who is legally separated from her "D"H and wants to take the DCs on holiday abroad. She has to get him to sign a form to allow her to do so, and I understand it has to be notarised. I should know this but just wanted to check - can the form be witnessed by a solicitor instead of a notary?

More generally, does anyone know where I can get full details of the form and the witnessing requirements? I have looked all over the internet and got nowhere, and it's virtually impossible to get through to the CAB on the phone!

For domestic purposes she merely needs the consent in writing. It doesn't need to be witnessed or notarised.

I couldn't possibly comment on the requirements of the country she's holidaying in, and for that she's best off speaking to their embassy in the UK.
