If you didnt agree a court ordering unsupervised contact, could you request that contact is supervised again due to certain concerns by way of a letter brought with me to court, or is there special form i need to fill in? What is the best way to approach it?

If a final order was made you need now to apply on form C100. If no final order has been made then you apply on form C2.
You can find the forms via google.

Ok, so can I take this form with me to court on the day. I have not been given a full bundle of up to date documents, I only lost legal aid the other day, and the hearing is this week. I have a day to prepare. So, if I say I'm requesting a change back to suprvised, because I have concerns, where/how do I then present this piece of paper. Do I have to pay for it?

Should I attach a statement to the above document, or is a position statement something I should just bring into the court room with me and hand to the judge. There are no orders for a statement, but in the past my solicitors have handed them in unrequested.

Is there anything else I need to take to court, or do at court to apply for a change of directions, back to supervised?

Should I even ask for time to prepare properly, or just deal with it, say I haven't got the evidence in front of me to support my request, but I do have it! I want this over with really, and not to delay any more, for my childrens sake, but I don't feel ready to fight this myself and I think more time would be helpful, catch 22.

Thought of some more questions overnight:-

What orders can the court can potentially make at an interim hearing. Either way, directed at myself, or at ex. ie, parenting course, psych evaluations, etc... or no contact, or transfer of residence etc... What applications can be made by either party with regards to the GAL.
