After separation what does this actually mean? Does it mean you have to agree? Simply consult, invite suggestions or does it potentially end up as a legal issue and in court if separated parents can't make a joint decision?

Parental Responsibility gives both parents equal responsibilities for a child although in most circumstances parents may act unilaterally. The exceptions to this are changing a child's name, taking children abroad and important educational and medical decisions.

When there is disagreement either parent may apply to court for an order to regulate PR ie contact or residence, specific issue order to resolve a specific issue or a prohibited steps order to prevent something from happening.

Yes, it does include choice of schools. Of course, if the child goes to state schools there may not be much choice.
