Endometrial hyperplasia - is lining measured whilst bleeding accurate?

Hi, would appreciate if anyone could shed some light!!

I’ve had irregular periods since I first started having them and was diagnosed with PCOS a few years ago. I recently went to a fertility clinic to talk about my irregular bleeding and lack of ovulation.
I hadn’t had a bleed for around four months (my last one was Provera induced). When they scanned me before starting a new course of Provera, my endometrial lining measured 7mm. The doctor prescribed Provera for 10 days (5mg twice daily) and told me to come back on day 3 of the next bleed to check if my lining had thinned. If it hadn’t, they said they would do a biopsy.

I went back while I was still bleeding, around day 4, and the lining measured 8.5mm. A different nurse did the scan and said it looked “smooth” with no clots or obvious abnormalities, so no biopsy was done at the time.

They’ve said they might rescan me a few days after the bleeding stops, about five days later, to see if the lining has gone down.
I’m confused and a bit anxious. Is it normal for the lining to still be 8mm while bleeding? Could that just be blood or tissue still being shed rather than new growth? Or is 8mm at this stage usually a sign of hyperplasia?

I’d really appreciate hearing from anyone who’s had something similar, especially anyone with PCOS who’s had thick linings after a Provera bleed.don’t know if that’s just my normal as I haven’t been scanned straight after a period for years and back then Inwas a lot slimmer and periods kind of regular. Thanks!

Hi! How are you going? I dont have pcos so I cant help you there but I do know so woman have thicker uterine lining then others. Mine is thick, when around ovulation my lining can reach 14mm. It can stay around 8mm during or after a cycle. Iv personally never been told i have anything wrong with my uterine lining and have never been sent to have tests. I do bleed heavier then most people and now since going through perimenopaus my lining isnt at thick which actually worries me a little. It was 12mm at my last ivf cycle before transfer and that transfer didnt work.

Hi there, thanks for replying! That’s really interesting and I wondered if some people might just have thicker linings than others. I think the rationale the fertility doc had was that since I don’t naturally ovulate on my own and go months without bleeding is my lining actually shedding. The fact my lining was only 7mm and at that point I hadn’t bled for 4 months didn’t seem too thick? Since I don’t have textbooks cycles, I’m not sure why they asked me to come in day 3/4 of my induced bleed to see if I had shed properly. I’m sure there was blood still in there ( I was actually bleeding during the scan) and that could’ve increased the lining to 8.5mm. But who knows.

They’ll be rescanning me again early next week to double check but still want to proceed with a pipelle biopsy. Guess I’ll see what their final decision is in a couple of days ! Best of luck with your future transfers x

Its good your specialist wants to explore all options so id go with what they are suggesting. But wanted to let you know that some woman do just naturally have thicker lining and everything be ok. Hopefully someone sees the post now its been bumped and can give you more info about the pcos and lining.
