What is a normal Antral Follicle Count (AFC)?

Hi all,

TTC and started with a fertility clinic. A little concerned as my Antral Follicle Count seemed high at 23 on one and 24 on the other ovary. Does this seem high? I'm early 30s. Curious to know what others have had as I'm sinking into panic mode!

Hi

I think there is quite a big range for 'normal' and it also varies widely based on age. I'm 33 and I think mine was about 18 for both sides together, but I ended up getting 26 eggs.

Did they test your AMH to see whether that aligns with the high AFC? Unless you have PCOS, I wouldn't be concerned about a high count. More eggs is not a bad thing. The only con I can think of is that your risk for OHSS will be slightly higher and as a result, you would possibly not be allowed to do a fresh transfer. But I see that many clinics only do frozen transfers so it might not even matter in your case.

Thanks for sharing @JJB92 ! Sounds like mine is a lot higher then, as combined I had 47 follicles D: I have had AMH tested but waiting blood results. I've had weirdly mixed views on whether I have PCOS, with doctors telling me different things and having varied results (sometimes androgen very high, sometimes low; sometimes multiple cysts, sometimes just one; not overweight/very healthy BMI, but have excessive hair; some years I'll have regular healthy cycles and some years it's irregular etc.), so the overriding impression I have is that it's borderline. In all likelihood I probably do have it, but a 'mild' form. I have been able to conceive but miscarried, so my concern is progesterone (I have a short luteal phase).

Yep, I am concerned about risk of OHSS if I take ovulation induction/stimulation treatments. Honestly terrified about it, but I'm assuming they start people on low doses and monitor? If you don't mind me asking, did you have any medications/injectables for ovulation?

Mild PCOS then probably explains the high AFC. What is your treatment plan? Ovulation induction or ivf?

I had ovulation induction with letrozole, trigger shots and additional progesterone before we eventually ended up doing IVF. They start you on the lowest dose and then monitor how your body responds. With IVF they will also monitor the follicle growth and estrogen levels.

I couldn't do a fresh transfer because my estrogen levels spiked too high indicating risk of OHSS, but was absolutely fine afterwards.
