5BB at age 38.5 - likelihood of healthy baby?

I know any information will be anecdotal but I'm an overthinker and like to have as much info as possible!

I'm trying to bank embryos with severely DOR.. I'm doing 3 cycles and usually only get 1/2 eggs a cycle.

I've done my first cycle and got 2 eggs, 1 blastocyst - 5BB. I'm 38.5 atm and will be doing PGT-A testing so I won't know if this one is even euploid until another 3 months or so down the line when they all get sent off to be tested. But just interested in others experiences, particularly around my age? How much does the grading at this stage mean?

The other embryo has not appeared to develop from 4 cells so suspect that one's out..

TIA

The grading doesnt mean much til pgta results

Im 38 too

We had 6 ' top quality ' blastocysts

4 were pgta abnormal

1 pgta normal

1 low level mosaic

Please stay strong, this is such a hard process. I had three cycles and got 40 eggs cross the three. Eggs don’t mean blasts unfortunately - we had 7 day 5 blasts and from that only three euploid embryos. What is amazing is that in the first cycle I only had one blast from 14 eggs (3BC and he was a euploid so is in the freezer) the second cycle was awful, a 3AB and aneuploid so we couldn’t have it. The last round gave us two euploid embryos.
The Embryo went in last week- a 4BB euploid and did really well up to that point, I’m waiting to do the final test next week and am so nervous. I’m 40 now and whilst the chances of having abnormalities is higher there is always a chance that your embryo will be fine. The grading really isn’t that important- it just helps to determine if it can withstand freezing and testing and helps them determine the best for transfer (or so I believe) different clinics also grade differently. but the PGTA is a lifesaver as that is what will help improve chances of a healthy pregnancy and baby by ruling out the abnormal ones which don’t have a good chance of taking. A top grade doesn’t mean a euploid unfortunately but a lower grade who is a euploid could be the one for you! A nurse once said to me, when we get pregnant naturally we don’t know the grade, they could be really poor but they survive because the uterus is great.
Keep positive- you have one gorgeous embryo and it could be absolutely fine x sending love x

did you know the grading of the pgt-a normal one, by any chance?

It was a day 4 top quality blastocyst

( blastocyst by day 4 )

Thats all i know

I wouldn't test them unless you are able to test them before freezing. Reason I say this is that after many unsuccessful transfers (had no problem making blasts) I was v v v tempted to defrost to test and find out what the issue might be. Opted to transfer the last one (which was something crap like a 4CC from memory) instead but in the morning they called to say it hadn't properly survived the thaw but they would give it a bit of time to wake up, and in the end they did transfer it. She's now my 3 year old daughter. Had I been defrosting to test, she would have been destroyed as they can only refreeze them if 75% or so of the cells survive intact.

On another note, I did test a whole cycle of 4 A grade embryos and they were all abnormal. They had high scores in Care Maps too (camera). They always say (and noone listens!) that the grading doesn't matter at all!

thanks... yes they are testing them fresh and then freezing.

Disappointing to hear that it doesn't really matter that the grading is decent! I thought it might be an indication of something positive for the embryo but I guess the genetic issues could still be there..

5BB is a solid grade.

Tha average euploid rate at 38 is around 40%.

TheFanciestPants · 19/06/2025 12:41

You are right. On average, higher graded blasts are more likely to be euploid. This study has details, it doesn't guarantee it but the odds are in your favour.

Wishing you all the best

Is that different to egg quality? My Dr said at my age the average number of good eggs is 60% which is substantially higher? (She said 40% of good quality eggs at 40...)

Following this, its really interesting. Im trying not to get into a Google black hole about it all!

sirensong · 19/06/2025 14:39

@Rosiestraws egg quality feeds into it but it's the blastocyst and euploid rate that matters.

The euploid rate under 35 is more like 55-60%. The reason the miscarriage rate isn't as high as 40-45% is because most aneuploid embryos don't implant in the first place, or are chemicals the majority may not notice.

Averages are just that though - individuals can do better or worse, including across their own rounds. On any given forum thread it's interesting to see real world examples but the actual studies and population level stats are far more representative because thousands of results are considered.

Transfered a 5BC and 3CC and it resulted in my perfect little boy . I was 38
