What tests after 2 failed FETs?

Hi everyone,

Would be very grateful for any guidance. I've just found out our second FET with a high-grade, euploid embryo on long protocol has failed. I'm devastated. For context, our first ever FET (with long protocol) worked and thankfully is now my beautiful son.

We have two more euploid embryos. While I would love to jump in and try the next embryo, I think it'd be wise to do some tests to investigate why this keeps happening. If anybody had faced a similar scenario, are there any tests you'd recommend asking for?

I’m in a similar position and my next step is hysteroscopy to check for any c-section scar defects. Obviously not sure if that applies to you but your clinic may have a view.

I don’t know if it’s bad luck or just a numbers game, but for us the successful FET was our best quality embryo so I wonder if that has any bearing.

Much less invasive than hysteroscopy- my clinic have advised me to take vitamin D supplement and check microbiome (I used Daye test which came back as ‘depleted’, need to re test soon).

When you say FET with long protocol, are you down regulating prior to FET? Would you try without?

I have actually put off the next FET as finding the process difficult and all the old feelings have resurfaced but I hope to do it in next few months.

Sorry to hear about your failed transfers.
I've done a saline ultrasound and the emma/alice test before my first FET with a euploid embryo. Results were fine but transfer failed.
My doctor then recommended I do a hysteroscopy, the NK cell blood test & NK uterine biopsy and potentially ERA. I think these are the standards tests they recommend for repeated failed implantation.
I declined to do any further tests and will just go ahead with my 2nd embryo transfer (my last euploid). The one extra thing they are adding this time is intrauterine hcg. Not sure if that helps or not but might be worth asking your clinic about it as it's fairly inexpensive and simple to do.
I am on a long protocol too. I asked if I could do natural or modified natural but they said they wouldn't recommend it as I have endo and adenomyosis as well. Not sure if you have similar reasons for the long protocol. If not, maybe trying natural next time might be an option.

@bubu24 do you mind me asking where your clinic is that they are doing such thorough testing? Did you do the nk cell test with your clinic? I’ve never heard of the hcg injection but very interested!

@TSHconfusion I'm with CRGH in London. I decided not to do the extra tests but if I wanted to, yes they would have been done at the clinic. I never heard of the hcg infusion before either so not sure if all clinics do it.
