IVFers fresh transfer - what day did you start progesterone?

Hello, I am wondering what day fellow fresh transfer IVFers started their progesterone?

It would be super helpful if you could add:

  • Which meds were you on?
  • Which clinic

My clinic started me on 2 days after transfer with Cyclogest pessaries.

I'm not sure now whether it was started too late. I'm trying to figure out where and what to do next as I think my IVF failed :(

Progesterone is normally started on the day of egg collection, in the evening when you get home after the procedure. 2 days after transfer is too late, with a day 5 blastocyst that's a week late.

At NHS I was on 2 x cyclogest pessaries a day (1 x morning, 1 evening) from the day of egg collection.

At ARGC to start with I was on 2 x cyclogest pessaries a day (1 x morning, 1 evening). After a couple of days this was increased to 4 x cyclogest pessaries a day (2 x morning, 2 evening) and 1 x lubion injection.

With a naturally conceived pregnancy the follicle that released the egg turns into the corpus luteum which produces progesterone. This happens pretty quickly and why you see a spike in temperature the next day after ovulation (progesterone raises temperature, estrogen lowers it). With IVF the follicles get damaged during the egg collection process and therefore cannot be relied upon to produce progesterone, this is why progesterone support is always required for IVF.

I would say the delay in starting progesterone is almost certainly the cause of your spotting and now bleeding. I note in your other thread you mention about complaining, I would submit a formal complaint to the clinic, then you can report them to the HFEA. Did you pay for the treatment on credit card? If so, you could request your credit card company cancel the transaction due to inadequate services received.

hi @ButterscotchBabe thanks for your response, but if i'm honest, your experience of ARGC is odd to me. x2 pessaries morning and evening is weird. Are you a representative from ARGC by any chance?

I don't think you should be giving "medical" information about progesterone on the forum when you are not a doctor. I didn't ask for your opinion about this.

To protect other women on here, I think it's important people be cautious about the posts that mention specific clinics and how amazing they are because this might be part of their marketing scheme. Of course, I might be wrong in this instance but your comment above is way too elaborate and tmi for a simple question I asked.

@ttconeyron wow what an ungrateful response. I am not a representative of ARGC, I've had one treatment cycle there and two at the NHS.

Over the years I've built up a wealth of knowledge on infertility and IVF, and was simply trying to help you with factual advice. Good luck with your clinic.

@ttconeyron I started day of egg collection progesterone 4X pessaries and one injection and this have been the same for my 3 previous rounds, maybe ask your clinic why they didn't start it until day of transfer, medication and protocol varies for each individual and also clinic, but I've never heard of someone starting 2 days after transfer

@ButterscotchBabe what time on the day of egg collection do you mind me asking? Did you take in the afternoon or evening? I did in the evening and my egg collection was the morning. Hoping that was okay!

I had 2 embryos transferred from fresh eggs but also used frozen eggs from a few years ago. Absolutely gutted as my younger eggs only yielded one low grade embryo to freeze - out of 11 eggs 7 fertilised and 1 low grade embryo! So I am really praying my fresh transfer went well and I have enough progesterone

I agree with the information from @ButterscotchBabe she is 100% correct with her information and answered your question perfectly. You took your progesterone too late. With ivf there is no guarentee the follicle can produce the progesterone as it did not go through the natural channels to ovulate therefore you need to build up progesterone either before egg collection or when eggs have been collected. If your cycle did not work it could or could not be due to low progesterone and you will need to talk to your clinic about changing your protocol. I hope you have all the success in the future with your fertility. And im sorry if your cycle didnt work.

Same here, NHS clinic with 2x cyclogest pessaries (1x morning, 1x evening) and they must be 12 hours apart! Starting in the evening after egg collection and continued on
