Recurrent miscarriage and IVF

Since 2022 I’ve had three missed miscarriages around 7 weeks, this past few weeks I had my third confirmed which was a twin IVF pregnancy. I’ve seen heartbeats on all my pregnancies but they all stopped developing around 7 weeks. I’m just looking for any information or guidance on next steps to look at. Labelled as unexplained infertility but have been told I have low AMH for my age (31). Currently waiting for recurrent miscarriage appointment through NHS before starting a second round of IVF. What tests should I be looking at? Happy to pay out private if needed, based in Merseyside. Or if anything has any good news stories to share I’d appreciate it. Thank you x

Have you both had a karotype blood test? Also I went here for this blood work.

I have similar MC; heartbeat detected at six weeks but then I’ve always gone onto lose symptoms and heartbeat/ growth has stopped.
Im so so sorry for your losses.

Have you had the POC tissue tested? We paid private for that.

We’re having chromosomal testing on the tissue from this pregnancy. Haven’t had any further blood tests other than the basics prior to IVF. I’ll definitely have a look at that clinic thank you!

I’m so sorry for your loss and everything you’ve been through.
I asked ChatGPT and this was their response, hope it helps x

Genetic & Chromosomal Testing

  1. Karyotyping (Parental Chromosome Testing) – Checks for balanced translocations or genetic abnormalities in the parents.
  2. PGT-A (Preimplantation Genetic Testing for Aneuploidy) – If using IVF, this can help determine if embryos have chromosomal abnormalities.
  3. Products of Conception (POC) Testing – If a loss occurs, testing the fetal tissue can help determine if chromosomal issues were the cause.

Hormonal & Endocrine Testing

  1. Thyroid Panel (TSH, Free T3, Free T4, Thyroid Antibodies) – Thyroid dysfunction (even mild) can contribute to pregnancy loss.
  2. Progesterone Levels – Low progesterone can lead to early pregnancy loss; supplementation may be needed.
  3. Prolactin – Elevated prolactin levels can interfere with implantation and early pregnancy maintenance.

Clotting & Immune System Tests

  1. Thrombophilia Panel – Includes tests for Factor V Leiden, Prothrombin gene mutation, Protein S/C deficiency, and Antithrombin III deficiency, which can cause clotting issues affecting placental blood flow.
  2. Antiphospholipid Syndrome (APS) Panel – Includes anticardiolipin antibodies, lupus anticoagulant, and beta-2 glycoprotein antibodies, which are linked to recurrent pregnancy loss.
  3. Natural Killer (NK) Cells / Immune Testing – Some believe an overactive immune response may contribute to losses, though this is a debated area.

Uterine & Structural Testing
10. Hysteroscopy / Saline Sonogram / MRI – Checks for uterine abnormalities (e.g., septum, fibroids, scarring).
11. Endometrial Receptivity Analysis (ERA) – Assesses if the uterus is receptive to implantation at the right time.
