Does this sound like silent endometriosis?

I’m 30 and have one living child, conceived when I was 26 (my husband was in his early 30s). We got pregnant the first month of trying.

We’ve now been trying for baby #2 for 1.5 years—one year of trying naturally, followed by six months of IVF. My first IVF round ended in a miscarriage at six weeks after a fresh transfer. Since then, I’ve been embryo banking and haven’t done any transfers yet, but I’m planning to in the next couple of months.

Embryo quality doesn’t seem to be the issue: our fertilization rate varies between 80–100%, and about 70% of our blastocysts have tested euploid. My hormone blood tests have all come back normal, and both ultrasounds and an HSG showed no issues.

My periods aren’t particularly painful, nothing that would raise red flags. I just don’t know what else to investigate. My doctor keeps saying EMMA/ALICE testing isn’t proven to be helpful, so I’m at a bit of a loss.

Could this be silent endometriosis? Or is there anything else I should be looking into?

Hi Sotif,

Just come across your thread looking for information on the EMMA and ALICE test.

I am in somewhat a similar boat. First child conceived when I was 29, first time. Now TTC for #2 at 33 and have had 4 miscarriages. Almost all of them conceived first time and all early losses, with slow progression on preg test (slow rising HCG). Nothing flagging in tests so i decided to take the Emma and Alice test (along with a hysteroscopy). I had had a v painful first miscarriage, and since then I've had bad cramps during ovulation, implantation and had a very heavy uterus feeling every time i've been pregnant since. (All that pain dismissed and not factored in test priority by my male doctors... of course)

2 days ago I had the results: it came back positive for an imbalance, that is causing endometritis. I have high Staphylococcus and Streptococcus and very low Lactobacillus. I am now taking Amoxicillin-Clavulanate for a week, as well as probiotics long term and will need a second biopsy to confirm that the microbiome is balanced.

Imbalances can be silent, and other than painful ovulation/implantation I've always had regular periods - so I would encourage you to test too. Although I would have thought the ERA test and EMMA/ALICE would routinely be done before IVF.

It was £1130 done at the Evewell, Hammersmith branch in London (+ £200 consultation before and again after) The clinic was recommended to me by a friend, but i found them a bit slow with results, and they are pricey. Looking to have my second biopsy somewhere else.

Thank you
I just had a look online - seems the clinic we are with - have it on their website as a possible add on..
But doesn't state price
